Texas Couple Killed After Small Plane Crashes Near Bogalusa Airport In Louisiana, US

collided in mid-air near Chilliwack Airport in British Columbia on August 3, 2026, prompting a multi-agency investigation by the Transportation Safety Board of Canada (TSB), Transport Canada and the Royal Canadian Mounted Police (RCMP). According to an RCMP statement issued on August 4 by Staff Sgt. Kris Clark, senior media relations officer with RCMP E Division, Chilliwack RCMP responded to reports of a mid-air collision at approximately 5:20 p.m. on Monday, August 3. "Members of the Chilliwack RCMP responded to a report of a mid-air collision between two planes that occurred in the Chilliwack area at approximately 5:20 p.m. on Monday, August 3," Clark said. Clark said Chilliwack RCMP, the Lower Mainland District Police Dog Services (LMD IPDS), Chilliwack Fire Department, and Chilliwack Search and Rescue (SAR) searched for the downed aircraft after receiving the report. "A small plane was located in a pond near Industrial Access Road in Chilliwack. The second plane was able to make it back safely to the airport. The occupants did not sustain injuries," Clark stated. The RCMP said its Underwater Recovery Team (URT) recovered the submerged aircraft and located the pilot. "The RCMP Underwater Recovery Team (URT) attended the scene and located one male deceased victim and the plane. Recovery efforts are currently underway," Clark said. The statement added that the Chilliwack RCMP General Investigation Services Team (GIST) is assisting federal investigators. "Chilliwack RCMP General Investigation Services Team (GIST) is currently supporting Transport Canada and the Transportation Safety Board (TSB) with their ongoing investigation. This multi-agency investigation is working to understand the circumstances that led to the collision," Clark said. The RCMP has not identified the deceased pilot, nor has it released a suspected cause of the collision. The Transportation Safety Board of Canada confirmed it has deployed an investigation team to Chilliwack following the occurrence. The agency said investigators would collect evidence and assess the accident involving two Cessna 172 aircraft. The TSB reiterated that its role is to advance transportation safety and that it does not assign fault or determine civil or criminal liability. Before the RCMP confirmed the fatality, Garry Atkins, manager of Chilliwack Airport, told The Canadian Press that the aircraft involved were operated by two local flight schools. He said one aircraft, carrying two occupants, landed safely at Chilliwack Airport, while the second aircraft, with one person on board, came to rest in a quarry lagoon after the collision. RCMP and Transport Canada have not publicly identified or confirmed the involvement of any specific flight schools at the time of reporting. As of August 5, investigators have not publicly determined what caused the two aircraft to collide. Pilot Dead After Two Cessna 172 Aircraft Collide Mid-Air Near Chilliwack Airport

Bimini, The Bahamas: A Tropic Ocean Airways seaplane carrying eight American passengers and two crew members crashed into the water while preparing for departure from North Bimini, Bahamas, on August 1, with all 10 occupants rescued safely in an incident that is now under investigation by Bahamian authorities. According to the Royal Bahamas Police Force, the Cessna 208B Grand Caravan seaplane was departing from North Bimini for Fort Lauderdale, Florida, when the accident occurred shortly before takeoff. Police said the aircraft was taxiing on the water and attempting to line up for departure when it reportedly veered uncontrollably to the left. The pilot is said to have taken evasive action to avoid heavy boat traffic in the area, but the aircraft struck a dock before overturning and sinking in waters estimated to be about 100 feet (30 metres) deep. The aircraft was operated by Tropic Ocean Airways, a Florida-based seaplane carrier that provides scheduled and charter flights between South Florida, the Bahamas and other Caribbean destinations. The airline confirmed the aircraft was involved in an accident and said it is cooperating fully with the ongoing investigation. All eight passengers, identified only as American tourists, along with the two crew members, escaped the aircraft and were rescued safely. Nick Pavlakis, general manager of Marine Max in Jupiter and a rescue boat operator, witnessed the crash and quickly took action to help the eight passengers and two crew members on board. Passenger Jeanne O'Brien, who was among those on board, told ABC News that the aircraft "went down pretty fast" after striking the dock. She said the passengers worked together to exit the cabin after the impact. Video recorded following the accident showed occupants in the water wearing life jackets as nearby vessels converged to assist. ABC News reported that the rescue was completed within minutes. Authorities have not determined the cause of the accident. Investigators are expected to examine the aircraft, pilot actions, operating conditions and other factors to establish why the seaplane lost directional control while taxiing for departure. No mechanical failure has been confirmed, and officials have not indicated whether weather or water conditions contributed to the accident. Tropic Ocean Airways said it is providing full assistance to investigators and remains focused on supporting those affected. The accident resulted in substantial damage to the aircraft, which came to rest submerged after overturning. 10 Rescued After Tropic Ocean Seaplane Crashes During Takeoff Near Bimini

Athens, Greece: A Danish pilot and a Greek fire service liaison officer were killed after two firefighting helicopters collided mid-air while battling a major wildfire west of Athens on Sunday, marking another tragedy as Greece faces multiple fast-moving wildfires amid extreme weather conditions. The collision occurred over the Psatha area near Porto Germeno, west of the Greek capital, where emergency crews were battling a wind-driven wildfire that has been burning since Friday. According to Greek authorities, both helicopters were Bell firefighting helicopters operating on aerial water-dropping missions over the fire zone. Each helicopter carried two crew members. One aircraft crashed following the collision, killing its Danish pilot and a Greek fire service liaison officer. The second helicopter managed to make an emergency landing, and its occupants, a British pilot and a Greek crew member survived with minor injuries. Both survivors were transported to hospital for treatment. Authorities have not yet released the identities of the deceased or the survivors. The exact cause of the mid-air collision remains under investigation. Officials have not indicated whether the helicopters made physical contact during water-dropping operations or while manoeuvring over the wildfire. No preliminary findings have been released, and investigators are expected to examine flight operations, communications and weather conditions in the area. The helicopters were supporting efforts to contain a rapidly spreading wildfire near Porto Germeno, a coastal community on the Gulf of Corinth. Strong, gusting winds significantly complicated firefighting operations, limiting aircraft movements and, at times, preventing helicopters and water bombers from safely collecting seawater for repeated drops. Nearly 500 firefighters supported by 23 aircraft were deployed to combat the blaze. Firefighters and municipal police officers were dispatched to extinguish the fire and conduct the initial emergency response, but there were no survivors. Footage circulating on social media showed smoke rising from the wreckage on the arid plains. The victims included seven Italian nationals, two Spanish nationals, and two German nationals, along with the two Peruvian flight crew, pilot Américo Salazar and co-pilot Irenka Guanilo del Carpio. Peru's Transport Ministry identified the crew members as pilot Américo Salazar and co-pilot Irenka Guanilo del Carpio. New Delhi, India: Akasa Air has backed the Centre's reported proposal to allow airport operators to own airlines, with Founder and Chief Executive Officer Vinay Dube expressing confidence that the government will introduce appropriate safeguards while framing the policy. At the same time, the airline is preparing for its next phase of expansion, with plans to explore an initial public offering (IPO) within the next two to five years as it continues to strengthen its financial position. Speaking in an interview with PTI, Dube said India requires greater competition in the aviation sector and that consumers would ultimately benefit from having more airline choices. His remarks come amid reports that the government is considering changes to existing norms that would permit airport operators to establish or own airlines. According to Dube, any framework allowing airport operators to own airlines would naturally involve regulatory and competitive considerations, particularly when a private entity controls both airport infrastructure and an airline. However, he expressed confidence that policymakers are fully aware of these issues and will incorporate the necessary safeguards while drafting the legislation. "I am very confident that the government will take into account the considerations involved when a private enterprise owns both an airport and an airline. Therefore, I am happy and supportive of this move," Dube told PTI. Emphasising the need for a more competitive aviation market, he said India can and should support multiple airlines to meet growing passenger demand. While acknowledging potential concerns surrounding ownership structures, he stressed that the larger objective should remain expanding consumer choice and encouraging healthy market competition. The comments come shortly after IndiGo Co-Founder and Managing Director Rahul Bhatia publicly warned that permitting airport operators to own airlines could create a "massive conflict of interest." A day later, the Adani Group, which currently operates eight airports across India, denied media reports suggesting it planned to enter the airline business. The policy discussion is unfolding as India continues to rank among the world's fastest-growing domestic aviation markets, with the government pursuing measures aimed at expanding connectivity, increasing airline capacity and encouraging new entrants. Akasa Air, which commenced commercial operations on August 7, 2022, will complete four years of operations this week. The airline currently operates a fleet of 40 Boeing 737 aircraft and has steadily expanded its domestic and international network since launch. The government's commitment to promoting competition was reiterated earlier this week when Civil Aviation Minister K. Rammohan Naidu, in a written reply to the Rajya Sabha on August 3, said the Centre remains committed to creating a conducive environment that supports the growth of existing airlines while facilitating the entry of new operators. In a separate interview in the past, Dube also outlined Akasa Air's long-term financial roadmap, revealing that the carrier intends to evaluate an initial public offering within the next two to five years. He said the airline remains financially healthy, describing Akasa Air as cash-positive and noting that its financial performance is progressing in line with management expectations. While confirming IPO plans are under consideration, Dube stressed that the airline will continue to prioritise sustainable expansion rather than pursuing aggressive fleet or network growth simply to increase its size."We will not chase growth for the sake of growth," Dube said. Akasa Air strengthened its financial position last year by raising fresh capital from investors, including Premji Invest and Claypond Capital, with the funds earmarked primarily for network and operational expansion. The airline has also outlined ambitious fleet expansion plans. Earlier, Chief Financial Officer Ankur Goel said Akasa Air aims to induct enough aircraft to build a fleet of 226 airplanes by the end of 2032, while continuing to improve financial performance and increase capacity. The accusations were announced by police on Thursday, August 6, 2026, after a two-year investigation into alleged malpractice, negligence, omissions and failures in the airline's operation and maintenance practices. Among those formally accused is Voepass owner and president José Luiz Felicio Filho. Federal police also accused 15 other current and former employees of endangering air traffic safety resulting in a fatal disaster, an offence that can carry a prison sentence of up to 12 years. Some of those accused additionally face allegations involving the omission of technical failures from aircraft logbooks. The 16 accused have not publicly commented on the police action, while prosecutors must now decide whether to bring formal criminal charges. The accident occurred on August 9, 2024, when Voepass Flight 2283 was operating from Cascavel in Paraná to São Paulo-Guarulhos. The ATR 72-500, registration PS-VPB, was carrying 58 passengers and four crew members. During the flight, the aircraft encountered severe icing conditions and progressively lost performance before entering an uncontrolled descent and flat-spin sequence. It fell about 13,000 feet and crashed into the grounds of a gated residential condominium in Vinhedo, killing everyone aboard. The latest criminal investigation has placed particular attention on what happened before the aircraft was dispatched for its final flight. Federal police investigator André Ribeiro said the ATR was irregularly cleared to depart despite known severe icing conditions and despite de-icing system failures occurring on three flights earlier that same day. Investigators also found from the aircraft's recorded data that the de-icing system had repeatedly failed or had been manually switched on and off during 136 flights. Ribeiro said the investigation represented a major turning point for Brazil's approach to aviation accidents and suggested that prosecutors should also examine possible responsibility involving Brazil's National Civil Aviation Agency, ANAC. The police investigation concluded that deliberate omissions, improper maintenance practices and organizational failures contributed directly to the decision to send the aircraft into dangerous weather. The new accusations follow the technical investigation already completed by Brazil's Center for Investigation and Prevention of Aeronautical Accidents, CENIPA. The Brazilian Air Force officially released the CENIPA final report on July 23, 2026, during a press conference in Brasília, after first presenting its findings privately to representatives and relatives of the victims. The 266-page report, identified as A-116/CENIPA/2024, was subsequently made available to the public through the CENIPA system. CENIPA did not assign criminal blame. Instead, its investigation reconstructed the technical, operational, human, organizational and regulatory factors that contributed to the accident. The report identified 19 contributing factors, including severe icing, failures involving the aircraft's de-icing system, crew actions, operational decision-making, maintenance shortcomings, organizational weaknesses and deficiencies in regulatory oversight. Earlier problems involving the aircraft's Airframe De-Icing system were not properly entered into maintenance records, while some maintenance activities were carried out without adequate supervision. CENIPA investigator Lieutenant Colonel Paulo Mendes Fróes said the company had developed a practice in which pilots and mechanics did not formally record aircraft problems so that aircraft could continue operating the following day. CENIPA said the crew's ability to process the warnings may have been affected by high workload and the phenomena known as “inattentional blindness” and “inattentional deafness”, in which critical information can fail to receive adequate attention despite being available to the crew. The investigation also identified weaknesses involving cockpit coordination, flight planning, pilot judgment, decision-making and the management of the aircraft's degraded performance. The investigation further found that the aircraft should not have been dispatched at the time of departure because of a fault involving its windshield-wiper system. CENIPA determined, however, that this particular defect did not directly contribute to the crash. More broadly, investigators found that maintenance practices and the airline's organizational processes allowed technical problems to remain unresolved or inadequately documented. ANAC's role also came under scrutiny in the CENIPA report. Investigators found that the regulator had previously identified weaknesses involving Voepass's maintenance controls, technical irregularities and other operational risks, but those findings did not translate into sufficiently effective risk-management measures to prevent the deterioration of the company's safety environment. ANAC said after receiving the report that it would conduct a detailed technical analysis of the findings and recommendations within its area of responsibility, with the review expected to take up to 120 days. CENIPA also issued safety recommendations extending beyond Voepass. One recommendation called for the European Union Aviation Safety Agency, EASA, to work with ATR on reviewing the aircraft's “Degraded Performance” procedure. Another recommendation concerned changing the “increase speed” alert from a caution-level alert to a warning-level alert, which would require immediate corrective action from the crew. With the Federal Police now having formally accused 16 people, the case moves into its next legal stage. The August 6 accusations therefore represent a separate development that follows the technical conclusions released by CENIPA on July 23. The accusations do not themselves constitute convictions or final criminal charges. The Federal Public Prosecutor's Office must determine whether the evidence warrants bringing charges against the accused. For the families of the 62 victims, however, the latest development marks a significant shift from determining what went wrong to examining who may ultimately be held legally responsible for allowing those failures to continue.

Turbulence has also emerged as a key concern, responsible for nearly three-quarters of serious injuries recorded last year. The Asia-Pacific and Europe/North Atlantic regions each reported three fatal accidents, while a single catastrophic crash in South America caused 62 deaths. Overall, Asia-Pacific recorded the highest number of fatalities, followed by South America, then Europe and the North Atlantic. Alongside accident data, ICAO flagged growing risks from Global Navigation Satellite System (GNSS) disruptions, such as jamming and spoofing, which threaten the reliability of modern navigation. The organization is updating guidance and working with member states to protect GNSS integrity, while also strengthening runway safety initiatives, turbulence monitoring, and wildlife strike reporting. ICAO is further developing frameworks for civil-military cooperation to manage risks linked to conflict zones and restricted airspace, and it is preparing safety systems for the integration of unmanned aircraft and advanced air mobility vehicles into shared skies. ICAO leaders underscored that the rise in accidents is a wake-up call for regulators, airlines, and governments worldwide. Caracas, Venezuela: Venezuelan authorities declared a nationwide state of emergency after two powerful earthquakes struck the country within seconds of each other on June 24, triggering widespread destruction across Caracas and several surrounding regions, forcing the closure of the nation's main international airport and launching a large-scale search-and-rescue operation. According to the United States Geological Survey (USGS), a magnitude 7.2 earthquake struck approximately 160 kilometers west of Caracas near Morón, followed just 39 seconds later by a stronger magnitude 7.5 tremor. Seismologists described the event as a rare seismic doublet. More than 20 aftershocks were recorded in the hours that followed. The earthquakes caused extensive damage across the capital and nearby states, with multiple residential and commercial buildings collapsing, roads and transportation infrastructure disrupted, and emergency crews deployed throughout affected areas. Image: BBC Video: FOX 4 Dallas-Fort Worth Vinhedo, Brazil: Brazilian federal police have formally accused 16 current and former Voepass employees over the August 2024 crash of an ATR 72-500 that killed all 62 people aboard, opening a new criminal-responsibility chapter in the investigation nearly two years after the aircraft fell in Vinhedo, São Paulo. The accusations were announced by police on Thursday, August 6, 2026, after a two-year investigation into alleged malpractice, negligence, omissions and failures in the airline's operation and maintenance practices. Among those formally accused is Voepass owner and president José Luiz Felicio Filho. Federal police also accused 15 other current and former employees of endangering air traffic safety resulting in a fatal disaster, an offence that can carry a prison sentence of up to 12 years. Some of those accused additionally face allegations involving the omission of technical failures from aircraft logbooks. The 16 accused have not publicly commented on the police action, while prosecutors must now decide whether to bring formal criminal charges. The accident occurred on August 9, 2024, when Voepass Flight 2283 was operating from Cascavel in Paraná to São Paulo-Guarulhos. The ATR 72-500, registration PS-VPB, was carrying 58 passengers and four crew members. During the flight, the aircraft encountered severe icing conditions and progressively lost performance before entering an uncontrolled descent and flat-spin sequence. It fell about 13,000 feet and crashed into the grounds of a gated residential condominium in Vinhedo, killing everyone aboard. The latest criminal investigation has placed particular attention on what happened before the aircraft was dispatched for its final flight. Federal police investigator André Ribeiro said the ATR was irregularly cleared to depart despite known severe icing conditions and despite de-icing system failures occurring on three flights earlier that same day. Investigators also found from the aircraft's recorded data that the de-icing system had repeatedly failed or had been manually switched on and off during 136 flights. Ribeiro said the investigation represented a major turning point for Brazil's approach to aviation accidents and suggested that prosecutors should also examine possible responsibility involving Brazil's National Civil Aviation Agency, ANAC. The police investigation concluded that deliberate omissions, improper maintenance practices and organizational failures contributed directly to the decision to send the aircraft into dangerous weather. The new accusations follow the technical investigation already completed by Brazil's Center for Investigation and Prevention of Aeronautical Accidents, CENIPA. The Brazilian Air Force officially released the CENIPA final report on July 23, 2026, during a press conference in Brasília, after first presenting its findings privately to representatives and relatives of the victims. The 266-page report, identified as A-116/CENIPA/2024, was subsequently made available to the public through the CENIPA system. CENIPA did not assign criminal blame. Instead, its investigation reconstructed the technical, operational, human, organizational and regulatory factors that contributed to the accident. The report identified 19 contributing factors, including severe icing, failures involving the aircraft's de-icing system, crew actions, operational decision-making, maintenance shortcomings, organizational weaknesses and deficiencies in regulatory oversight. Earlier problems involving the aircraft's Airframe De-Icing system were not properly entered into maintenance records, while some maintenance activities were carried out without adequate supervision. CENIPA investigator Lieutenant Colonel Paulo Mendes Fróes said the company had developed a practice in which pilots and mechanics did not formally record aircraft problems so that aircraft could continue operating the following day. CENIPA said the crew's ability to process the warnings may have been affected by high workload and the phenomena known as “inattentional blindness” and “inattentional deafness”, in which critical information can fail to receive adequate attention despite being available to the crew. The investigation also identified weaknesses involving cockpit coordination, flight planning, pilot judgment, decision-making and the management of the aircraft's degraded performance. The investigation further found that the aircraft should not have been dispatched at the time of departure because of a fault involving its windshield-wiper system. CENIPA determined, however, that this particular defect did not directly contribute to the crash. More broadly, investigators found that maintenance practices and the airline's organizational processes allowed technical problems to remain unresolved or inadequately documented. ANAC's role also came under scrutiny in the CENIPA report. Investigators found that the regulator had previously identified weaknesses involving Voepass's maintenance controls, technical irregularities and other operational risks, but those findings did not translate into sufficiently effective risk-management measures to prevent the deterioration of the company's safety environment. ANAC said after receiving the report that it would conduct a detailed technical analysis of the findings and recommendations within its area of responsibility, with the review expected to take up to 120 days. CENIPA also issued safety recommendations extending beyond Voepass. One recommendation called for the European Union Aviation Safety Agency, EASA, to work with ATR on reviewing the aircraft's “Degraded Performance” procedure. Another recommendation concerned changing the “increase speed” alert from a caution-level alert to a warning-level alert, which would require immediate corrective action from the crew. With the Federal Police now having formally accused 16 people, the case moves into its next legal stage. The August 6 accusations therefore represent a separate development that follows the technical conclusions released by CENIPA on July 23. The accusations do not themselves constitute convictions or final criminal charges. The Federal Public Prosecutor's Office must determine whether the evidence warrants bringing charges against the accused. For the families of the 62 victims, however, the latest development marks a significant shift from determining what went wrong to examining who may ultimately be held legally responsible for allowing those failures to continue.
