The incident involved IndiGo Flight IGO2113, an Airbus A321 operating from Delhi to Hyderabad, and IndiGo Flight IGO2206, an Airbus A320 operating from Delhi to Raipur. In addition, monitoring during the climb phase was insufficient, leading to reduced vertical and lateral separation from IGO2206. The bureau also noted that six previous SID deviation events involving IndiGo aircraft between August and November 2023 had not been reported through mandatory safety reporting channels, highlighting weaknesses in both the airline’s internal safety culture and compliance with regulatory requirements. In response to these findings, the AAIB has recommended that the DGCA carry out a one-time inspection of the safety management systems of all scheduled operators and the Air Navigation Service Provider. The goal is to evaluate the effectiveness of hazard identification, risk assessment, and reporting culture, as well as compliance with mandatory occurrence reporting. The AAIB has also emphasized that airlines must ensure that all previous and future incidents are reported promptly and accurately to both the DGCA and the AAIB. The Airports Authority of India (AAI) has been advised to review ATC procedures, particularly during high-intensity simultaneous departures, and to assess the Short Term Conflict Alert (STCA) system and airspace design to prevent future separation breaches. IndiGo has been recommended to take corrective action by ensuring accurate entries into the Flight Management and Guidance System (FMGS), minimizing cockpit disturbances during pre-flight setup, and strengthening cross-checks during training. Aviation safety experts note that these recommendations are critical for maintaining operational safety, especially as India’s air traffic continues to grow rapidly. The AAIB’s recommendations focus on the importance of robust safety management system audits, procedural discipline, and a culture of compliance to prevent near-misses from escalating into accidents. The DGCA is expected to follow up on the AAIB’s recommendations, potentially conducting targeted inspections across airlines and air navigation services to identify and rectify systemic vulnerabilities. The incident and the bureau’s final report highlight the vital role of near-miss investigations in reinforcing aviation safety, ensuring that lessons from minor incidents are institutionalized and preventive measures are implemented effectively.

According to a regulatory filing by InterGlobe Aviation, the parent company of IndiGo, the Directorate General of Civil Aviation (DGCA) found the airline in breach of rules after it implemented a Standard Instrument Departure (SID) and Instrument Flight Procedure (IFP) that had been internally formulated by the company rather than issued by the Airport Authority of India (AAI), the only authorised body to publish such procedures. The DGCA stated that the airline’s actions violated Rule 133A of the Aircraft Rules, 1937, along with provisions of Civil Aviation Requirement (CAR) Section 9, Series C, Part I, specifically paragraphs 2.2 and 2.34, which mandate strict adherence to officially promulgated instrument procedures. InterGlobe confirmed that the communication from the DGCA was received on 17 November 2025. In its exchange filing, the company noted that the penalty would have “no material impact” on its operations or financial position. Industry observers say the latest order reflects DGCA’s tightening oversight on airlines’ compliance with flight procedures, especially at airports that require strict navigational conformity. Instrument departures and arrivals are critical for maintaining flight safety, particularly in regions with terrain challenges or high-density traffic. The penalty comes weeks after DGCA imposed a ₹40 lakh fine on IndiGo for using non-qualified full-flight simulators to train pilots operating to Category C airports, including Leh, Calicut, and Kathmandu. The investigation revealed that nearly 1,700 pilots underwent training on simulators that did not meet regulatory standards prescribed for operations into these high-difficulty airports. In that case, DGCA penalised IndiGo’s Director of Training and Director of Flight Operations with individual fines of ₹20 lakh each, citing oversight lapses and deviations from CAR guidelines.

New Delhi, India: More than 800 flights were delayed at Indira Gandhi International Airport (IGIA) after a technical failure disrupted air traffic control systems on Friday, causing widespread operational chaos at one of the world’s busiest airports. The Airports Authority of India (AAI) confirmed that a malfunction occurred in the airport’s Automatic Message Switching System (AMSS) , a critical platform that generates and distributes flight plans to controllers and pilots. The failure forced the ATC to manually process flight plans, drastically slowing departures through Friday. “The system developed a technical issue late Thursday evening, leading to temporary delays in flight departures,” the AAI said in a statement on X (formerly Twitter). “Technical teams are working to restore the system at the earliest.” According to flight-tracking platform Flightradar24, over 800 flights experienced delays, with more than 175 departures affected on Friday morning alone. The average delay was around 55 minutes, but several international routes were held back much longer. Domestic carriers, including IndiGo, SpiceJet and Air India, reported schedule disruptions. IndiGo’s shares dropped 1.5 % in response. An ITA Airways flight to Rome was delayed by nearly two hours, while a Virgin Atlantic service to London took off more than an hour behind schedule. While the AAI has not confirmed the root cause, officials said preliminary checks are underway to determine whether the issue was linked to a possible malware intrusion. A senior aviation ministry source told Reuters that “cybersecurity experts are reviewing the system logs for any signs of tampering or external interference.” Delhi’s IGIA, managed by GMR Airports Limited and overseen by the AAI, handled 78 million passengers in 2024, ranking as the ninth-busiest airport globally according to Airports Council International. AAI technicians and software vendors were still working late Friday to fully restore the AMSS. The Directorate General of Civil Aviation (DGCA) has been briefed and is monitoring the situation for compliance and safety implications. No cancellations were officially reported, but passengers experienced long queues, rescheduling, and confusion as airlines adjusted flight plans.
