The incident involved IndiGo Flight IGO2113, an Airbus A321 operating from Delhi to Hyderabad, and IndiGo Flight IGO2206, an Airbus A320 operating from Delhi to Raipur. The two aircraft came dangerously close to each other shortly after departure, breaching the minimum separation standards mandated for controlled airspace. Fortunately, the Traffic Collision Avoidance System (TCAS) alerts helped pilots maintain safe separation, and no injuries or damage to the aircraft were reported. The AAIB’s final report identified multiple factors that contributed to the near-miss. The crew of IGO2113 failed to correctly set up and verify the Standard Instrument Departure (SID) data, which is critical for maintaining a safe climb trajectory. In addition, monitoring during the climb phase was insufficient, leading to reduced vertical and lateral separation from IGO2206. The bureau also noted that six previous SID deviation events involving IndiGo aircraft between August and November 2023 had not been reported through mandatory safety reporting channels, highlighting weaknesses in both the airline’s internal safety culture and compliance with regulatory requirements. In response to these findings, the AAIB has recommended that the DGCA carry out a one-time inspection of the safety management systems of all scheduled operators and the Air Navigation Service Provider. The goal is to evaluate the effectiveness of hazard identification, risk assessment, and reporting culture, as well as compliance with mandatory occurrence reporting. The AAIB has also emphasized that airlines must ensure that all previous and future incidents are reported promptly and accurately to both the DGCA and the AAIB. The Airports Authority of India (AAI) has been advised to review ATC procedures, particularly during high-intensity simultaneous departures, and to assess the Short Term Conflict Alert (STCA) system and airspace design to prevent future separation breaches. IndiGo has been recommended to take corrective action by ensuring accurate entries into the Flight Management and Guidance System (FMGS), minimizing cockpit disturbances during pre-flight setup, and strengthening cross-checks during training. Aviation safety experts note that these recommendations are critical for maintaining operational safety, especially as India’s air traffic continues to grow rapidly. The AAIB’s recommendations focus on the importance of robust safety management system audits, procedural discipline, and a culture of compliance to prevent near-misses from escalating into accidents. The DGCA is expected to follow up on the AAIB’s recommendations, potentially conducting targeted inspections across airlines and air navigation services to identify and rectify systemic vulnerabilities. The incident and the bureau’s final report highlight the vital role of near-miss investigations in reinforcing aviation safety, ensuring that lessons from minor incidents are institutionalized and preventive measures are implemented effectively.

According to a letter from Transport Canada Civil Aviation (TCCA) to Air India officials, the Royal Canadian Mounted Police (RCMP) reported that the pilot, identified as Captain Saurabh Kumar, reported for duty while under the influence of alcohol and was deemed unfit for the flight. Canadian authorities conducted two breathalyser tests that confirmed the presence of alcohol, following which the pilot was advised to leave the aircraft and was subsequently removed from duty at the scene. Transport Canada has characterised the occurrence as a violation of Canadian Aviation Regulations (CARs), citing breaches of sections 602.02 and 602.03, which govern crew fitness for duty and alcohol consumption limits, as well as conditions of Air India’s Foreign Air Operator Certificate (FAOC). In its correspondence, Transport Canada has formally asked Air India to investigate the matter under its Safety Management System and submit a detailed corrective action report by January 26, 2026. The regulator indicated that enforcement action by both the RCMP and the TCCA is likely as the review progresses. Air India confirmed that Flight AI186 experienced a last-minute delay on December 23 after the cockpit crew member was offloaded prior to departure “based on concerns raised by the Canadian authorities regarding the pilot’s fitness for duty.” The airline said an alternate pilot was rostered in accordance with safety protocols, resulting in the delayed departure. In an official statement, Air India said it regrets the inconvenience caused to passengers, is fully cooperating with Canadian authorities, and has taken the pilot off flying duties pending the outcome of the inquiry. The airline reiterated a zero-tolerance policy toward violations of applicable rules and regulations, noting that “any confirmed violation will attract strict disciplinary action in line with company policy.” Initial accounts of the incident suggest that the episode first came to light after a duty-free shop employee at the Vancouver airport reported a concern about the pilot either smelling of alcohol or consuming it moments before boarding preparations. The Boeing 737 aircraft was diverted to Kochi as a precaution after the flight crew received an alert indicating a technical issue related to the landing gear. Emergency response protocols were immediately activated at the airport, and the aircraft landed safely without incident. All passengers and crew disembarked safely, with no injuries reported. Airport authorities said fire and rescue services were deployed along the runway as part of standard safety measures. Post-landing inspections confirmed damage to one of the aircraft’s tyres, following which the aircraft was grounded for detailed technical checks. Air India Express said passenger safety remained its top priority and that alternative arrangements were made to transport passengers to Kozhikode. Seoni, India: A training aircraft belonging to Redbird Flight Training Academy crashed on Monday evening after its wing struck a 33 kV high-voltage power line near Sukatra village, in the Kurai police-station area of Seoni district. According to police, the incident occurred around 6:25–6:30 pm, roughly 2 km from the Suktara airstrip, as the aircraft was preparing to land. Eyewitness accounts describe sparks and a loud explosion as the plane’s wing contacted the line, after which the aircraft plunged into a nearby field. Authorities identified those on board as the instructor pilot Ajit Anthony and trainee pilot Ashok Chawda. Both sustained injuries and were taken to a private hospital in Barapathar. Officials say they are “out of danger.” Local police and power-company staff, aided by villagers, rescued the two pilots from the wreckage soon after the crash. A preliminary probe indicates that the aircraft may have suffered a “technical snag,” possibly an engine malfunction, causing it to lose power as it descended. The wing then struck the overhead power line of the Badalpar Substation, triggering the crash. The impact caused the 33 kV line to snap, immediately tripping the electricity supply. Roughly 80–90 villages around the substations reportedly experienced a blackout, and repair crews have been deployed to restore power to the affected areas. The district police have launched an official investigation to ascertain the precise cause of the crash. It remains uncertain whether pilot error, mechanical failure, or a combination of factors led to the collision. Final determination will await the outcome of the inquiry. So far, there have been no fatalities. Residents near the Suktara airstrip, however, expressed deep concern, noting that this was not the first incident involving a training aircraft in the area. Earlier in May 2025, a training aircraft operated by Redbird Aviation Pvt. Ltd. overturned during landing at the Suktara airstrip in Seoni district, Madhya Pradesh. The flight was being conducted solo by a trainee pilot. Initial reports attribute the incident to a faulty landing. Fortunately, the trainee escaped without injury and there was no fire. Months later on August 9, 2025 again a trainer aircraft of Redbird Flight Training Centre crash-landed near Baramati Airport in Pune district. During a routine training sortie at around 8 a.m., the trainee pilot realised one of the aircraft’s tyres was damaged. Upon touchdown, the front wheel detached and the aircraft veered off the taxiway, crossing to the far side of the airfield. Despite the abrupt landing and loss of the wheel, the pilot escaped unhurt.

The pilot and his wife, who was the passenger, were able to evacuate the plane on their own. Both individuals were conscious and alert when first responders arrived at the scene. They were subsequently transported to Sacred Heart Hospital in Pensacola for evaluation and treatment. The crash site was secured by Gulf Shores Police and Fire Rescue, as well as airport management. Fortunately, the incident occurred outside the airport boundaries, so airport operations were not affected, and traffic on Alabama 59 remained unaffected.
