New Delhi, India: India’s Aircraft Accident Investigation Bureau (AAIB) has recommended a comprehensive one-time inspection of airlines’ safety management systems (SMS) by the Directorate General of Civil Aviation (DGCA), following its 35-page final investigation report into a serious air proximity incident involving two IndiGo aircraft in Delhi airspace on November 17, 2023. The incident involved IndiGo Flight IGO2113, an Airbus A321 operating from Delhi to Hyderabad, and IndiGo Flight IGO2206, an Airbus A320 operating from Delhi to Raipur. The two aircraft came dangerously close to each other shortly after departure, breaching the minimum separation standards mandated for controlled airspace. Fortunately, the Traffic Collision Avoidance System (TCAS) alerts helped pilots maintain safe separation, and no injuries or damage to the aircraft were reported. The AAIB’s final report identified multiple factors that contributed to the near-miss. The crew of IGO2113 failed to correctly set up and verify the Standard Instrument Departure (SID) data, which is critical for maintaining a safe climb trajectory. In addition, monitoring during the climb phase was insufficient, leading to reduced vertical and lateral separation from IGO2206. The bureau also noted that six previous SID deviation events involving IndiGo aircraft between August and November 2023 had not been reported through mandatory safety reporting channels, highlighting weaknesses in both the airline’s internal safety culture and compliance with regulatory requirements. In response to these findings, the AAIB has recommended that the DGCA carry out a one-time inspection of the safety management systems of all scheduled operators and the Air Navigation Service Provider. The goal is to evaluate the effectiveness of hazard identification, risk assessment, and reporting culture, as well as compliance with mandatory occurrence reporting. The AAIB has also emphasized that airlines must ensure that all previous and future incidents are reported promptly and accurately to both the DGCA and the AAIB. The Airports Authority of India (AAI) has been advised to review ATC procedures, particularly during high-intensity simultaneous departures, and to assess the Short Term Conflict Alert (STCA) system and airspace design to prevent future separation breaches. IndiGo has been recommended to take corrective action by ensuring accurate entries into the Flight Management and Guidance System (FMGS), minimizing cockpit disturbances during pre-flight setup, and strengthening cross-checks during training. Aviation safety experts note that these recommendations are critical for maintaining operational safety, especially as India’s air traffic continues to grow rapidly. The AAIB’s recommendations focus on the importance of robust safety management system audits, procedural discipline, and a culture of compliance to prevent near-misses from escalating into accidents. The DGCA is expected to follow up on the AAIB’s recommendations, potentially conducting targeted inspections across airlines and air navigation services to identify and rectify systemic vulnerabilities. The incident and the bureau’s final report highlight the vital role of near-miss investigations in reinforcing aviation safety, ensuring that lessons from minor incidents are institutionalized and preventive measures are implemented effectively.

The aircraft, carrying 173 passengers, departed from Paris-Orly and climbed to cruising altitude when crew detected a technical issue that prompted them to declare an emergency and reroute to the nearest suitable airport as a precautionary measure, according to Air France’s statement. The airline described the fault as a technical problem that did not compromise the aircraft’s airworthiness, emphasizing that crews followed procedures by shutting down the affected engine and diverting safely. The aircraft touched down at Lyon at approximately 18:25 local time, with no injuries reported. According to aviation specialists cited by French media, the incident likely involved a “pompage moteur”, a known phenomenon where air pockets in the engine cause significant vibrations, forcing pilots to shut down the engine. Once the engine was cut, vibrations subsided and the Airbus A320 continued to land normally on the remaining operational engine. Several passengers described a frightening experience in the cabin, reporting strong vibrations, alarms activating, and what they perceived as flashes or flames near one wing. Some brief flames at the engine exhaust area limited in duration and contained within the engine could explain eyewitness descriptions but did not indicate an in-flight fire event. After the unscheduled landing in Lyon, ground teams accommodated all passengers and arranged onward travel. Air France rebooked the 173 passengers on a later flight from Lyon to Bastia, which departed at 22:39 and arrived at 23:50. From Bastia, passengers were transferred by bus to Ajaccio, arriving in the early hours of Sunday morning. The Boeing 737 aircraft was diverted to Kochi as a precaution after the flight crew received an alert indicating a technical issue related to the landing gear. Emergency response protocols were immediately activated at the airport, and the aircraft landed safely without incident. All passengers and crew disembarked safely, with no injuries reported. Airport authorities said fire and rescue services were deployed along the runway as part of standard safety measures. Post-landing inspections confirmed damage to one of the aircraft’s tyres, following which the aircraft was grounded for detailed technical checks. Air India Express said passenger safety remained its top priority and that alternative arrangements were made to transport passengers to Kozhikode. Reports indicate that the lower left‑hand winglet sustained damage. flydubai swiftly confirmed that all passengers and crew disembarked safely, with no injuries, fuel spillage or fire risk reported. The airline said affected travellers have been re‑accommodated on the next available flights while the damaged aircraft undergoes inspection and repair. The next scheduled leg, a flight from Malé to Colombo, was cancelled as a result of the incident. Authorities at the Maldives airport are expected to launch a formal investigation into the incident, focusing on apron safety protocols, ground‑handling procedures and vehicle movement controls around parked aircraft. flydubai reiterated that the “safety of our passengers and crew remains our highest priority at all times.” The aircraft will remain grounded until repairs are completed and regulatory standards for airworthiness are satisfied. On 28 November 2025 a Emirates Airbus A380‑842 at Manchester Airport (UK) was struck by a fuel‑truck while parked at a gate, during routine turnaround after arrival from Dubai. The collision damaged the cowling of the engine intake though there was no fuel leak, fire or injuries and forced cancellation of the return flight EK18 to Dubai. The aircraft remained grounded for inspections and repairs, and took off again on 30 November for flight EK20 to Dubai, signalling the damage was deemed repairable.

Airlines including Air India, Akasa Air and IndiGo cancelled or rerouted multiple flights as the Directorate General of Civil Aviation (DGCA) issued an urgent advisory enforcing strict avoidance of ash-affected airspace. The DGCA directed airlines to steer clear of regions and flight levels identified as contaminated by volcanic ash. The regulator also instructed carriers to reassess flight plans, fuel requirements and alternates, and to file immediate reports of any suspected ash encounters. The chartered aircraft, an Embraer ERJ-145 operated by Airjet Angola (registration D2-AJB), was on a flight (MBC-100) from Lubumbashi to Kolwezi when it veered off runway 29 during landing and caught fire. According to the bureau of aircraft accident archives, both main landing gears were reportedly sheared off as the airplane came to a halt, and the tail section ignited. All 26 passengers and 3 crew evacuated; there are no reported fatalities or serious injuries. Initial reports suggest the aircraft touched down before a displaced threshold on runway 29, which was under maintenance. The Notam issued for Kolwezi Airport indicated that 1,000 metres of the runway were being resurfaced, reducing the effective landing distance. According to the Bureau of Aircraft Accidents Archives, this could have contributed to the gear collapse. Eyewitnesses and social media footage show thick smoke rising from the rear fuselage as passengers escaped, some via overwing exits. Isaac Nyembo, communications adviser to Minister Kabamba, said around 20 people were evacuated before the fire spread, though luggage was damaged. Concerns have also been raised over emergency response readiness at Kolwezi Airport. No fire trucks arrived for several minutes, despite the airport being categorized to require rapid rescue capability. Minister Kabamba and his delegation were heading to Kalondo, near Kolwezi, following a recent mining accident in which over 30 workers died in a bridge collapse. The delegation was expected to assess the damage, console affected families, and coordinate government support. An investigation has been launched by the DRC’s aviation authority (BPEA) into the cause of the runway excursion and fire. Kolwezi Airport, located in Lualaba Province, has a single asphalt runway (11/29), around 2,410 metres long. Aviation experts have pointed out that infrastructure challenges, such as runway maintenance, coupled with limited firefighting capacity, may pose ongoing safety risks.

The helicopter, registered RA-19307, was flying from Kizlyar to Izberbash on November 7 when it reportedly caught fire while airborne near the village of Achi-Su, close to the Caspian Sea. videos circulating on Russian and international media show the aircraft’s tail section detaching as it spun uncontrollably before slamming into the ground and erupting into flames. According to a statement by the plant, the dead included KEMZ’s deputy general director, chief engineer, and chief designer. Two other employees who were initially reported injured also succumbed to their injuries, KEMZ, part of the Rostec defense conglomerate, manufactures the Ka-226 multipurpose helicopter used by Russian armed forces and civilian operators. Video evidence reviewed by multiple outlets shows the pilot struggling to stabilize the aircraft after its tail rotor failed mid-air. Witnesses reported seeing the helicopter hover momentarily before the fuselage ignited and fell to the ground. The aircraft crashed into an unoccupied private house, causing no ground casualties, regional authorities confirmed. The same model was previously involved in several non-fatal incidents linked to gearbox and rotor-system issues.
