London, United Kingdom: The UK Civil Aviation Authority (CAA) has formally asked Air India to provide a detailed explanation of an incident involving a Boeing 787-8 Dreamliner that departed London Heathrow Airport on February 1 despite a reported anomaly with a critical aircraft system. The regulator has warned the airline that failure to respond fully within one week could trigger regulatory action affecting its Dreamliner operations. Air India flight AI132, operating a Boeing 787-8 Dreamliner registered VT-ANX, departed Heathrow for Bengaluru after pilots noted irregular behaviour with a fuel control switch, a vital cockpit control that regulates fuel flow to the engines. During engine start, one of the two fuel control switches did not remain positively latched in the “RUN” position on the first two attempts before stabilising on the third. The flight continued without in-air issues and landed in Bengaluru. Upon arrival, the crew filed a defect report citing the anomaly, and the aircraft was grounded for inspection on February 2. The UK Civil Aviation Authority has taken the step of demanding a formal response from Air India, including: A comprehensive account of pre-flight maintenance actions taken before departure, A root-cause analysis of the fuel control switch event, A preventive action plan to reduce the risk of recurrence across Air India’s Dreamliner fleet, which comprises 33 aircraft. In official correspondence, the CAA emphasised that regulators routinely seek detailed explanations following incidents affecting safety-critical components, particularly when an aircraft departs with a potential defect. Air India said it conducted precautionary re-inspections of fuel control switches across its Boeing 787 fleet after the incident and reported no issues found during these checks. The airline acknowledged the Directorate General of Civil Aviation (DGCA) oversight and said it will comply with the regulator’s guidance to circulate Boeing’s recommended operating procedures for the switches to all flight crews. India’s DGCA also issued a clarification, stating that post-flight inspections determined the fuel switches were serviceable and functioning normally. According to the regulator, the switch may move if light pressure is applied in an incorrect direction, but its design does not indicate a defect when used according to approved procedures. The DGCA advised Air India to reinforce correct switch handling among pilots. Fuel control switches are crucial for aircraft safety. They allow pilots to start and shut down engines and are designed with locking mechanisms to prevent unintended movement. This category of component has been under heightened scrutiny following the June 2025 crash of an Air India Boeing 787-8 in Ahmedabad, where preliminary investigation reports indicated fuel control switches played a role in dual-engine power loss shortly after takeoff. That accident, which resulted in significant loss of life, triggered industry-wide inspections and close regulatory observation of Boeing 787 systems. Though the final accident report remains pending, aviation authorities worldwide have been monitoring similar technical aspects, including fuel system controls, in current operations. Air India faces a deadline to submit its response to the UK CAA within one week of the February 4 notice. The airline has stressed safety as its utmost priority and reiterated cooperation with international regulators and Boeing in evaluating the matter.

The Swiss Transportation Safety Investigation Board (STSB) released its final report this month, saying the airline’s inaction was “incomprehensible from a safety perspective,” after a faulty component on an Airbus A330 forced pilots to execute an emergency descent and divert back to Zurich Airport in September 2024. On September 13, 2024, an Airbus A330 registered HB-JHI operating flight LX18 from Zurich to Newark Liberty International Airport encountered a critical pressurisation problem shortly after take-off. During ascent, the aircraft began losing cabin pressure due to a defective component within the pressurisation system, a “skin check valve” which failed to seal properly. Although both outflow valves indicated “fully closed,” the aircraft was unable to build sufficient differential cabin pressure. According to the STSB report, cockpit instruments showed inadequate pressurisation despite normal system settings. Cockpit crews donned oxygen masks, manually deployed passenger oxygen systems, and initiated an emergency descent at approximately 5,000 feet per minute before returning safely to Zurich. All 205 passengers and 12 crew members evacuated normally, and no injuries were reported. The STSB’s investigation found that the root cause of the pressurisation failure was a mechanically compromised skin check valve that had not been replaced in over eight years, despite Airbus issuing a service bulletin in 2016 recommending its earliest possible replacement and regular inspections every 24 months. The manufacturer’s bulletin identified that the original valve design was prone to hinge wear, vibration damage and spring failure, all of which could lead to air loss and potential depressurisation events during flight. European aviation regulators and maintenance manuals require that such service bulletins be carefully considered by operators to ensure fleet safety. Investigators noted that the defective valve on the SWISS aircraft remained in service far beyond its recommended inspection and replacement interval, even after being removed and inspected multiple times without upgrading to the redesigned part. The report concluded that this prolonged delay in corrective maintenance directly contributed to the in-flight incident. In its final recommendations, the STSB urged the European Union Aviation Safety Agency (EASA) to mandate the skin check valve replacement on all applicable Airbus A330 and A340 aircraft via a formal Airworthiness Directive rather than relying on voluntary compliance with service bulletins. The board cautioned that gradual decompression events can elude crew awareness in early stages, increasing risk if they occur at cruising altitudes. SWISS International Air Lines, part of the Lufthansa Group, had not issued an immediate public response, and the airline had not yet provided a formal comment in response to the STSB’s final report.

The attendant sought medical treatment upon arrival at Taoyuan Airport on September 25 and was initially treated at Chang Gung Memorial Hospital in Linkou. She was later transferred to China Medical University Hospital in Taichung, where she passed away on October 10. Reports indicate that Sun had informed her cabin chief about receiving an injection at the airport for pain and fever before being referred to a hospital's rheumatology department. Doctors suspected Adult-Onset Still’s Disease (AOSD), a rare autoimmune disorder characterized by high fevers, salmon-colored rashes, sore throat, joint pain, and swollen lymph nodes. The condition affects roughly 1 in 100,000 people worldwide and can lead to severe complications such as pericarditis, pneumonia, or macrophage activation syndrome, which carries a high mortality rate. The Taoyuan Flight Attendants Union has called for better care and support for sick flight attendants, urging EVA Air to ease sick leave rules. EVA Air has launched an internal investigation, and Taiwan labor authorities are also examining the circumstances surrounding Sun’s death. EVA Air is currently facing a public relations fallout on multiple fronts.
