According to the FAA, the airline failed to conduct required follow-up drug and alcohol testing for 11 employees, including pilots, flight attendants, and aircraft mechanics, who had previously tested positive for prohibited substances. The agency stated that these employees had tested positive for substances including marijuana, cocaine, and amphetamines, triggering strict federal requirements for return-to-duty clearance and long-term follow-up testing. However, investigators allege that Southwest allowed the affected personnel to resume safety-sensitive duties without completing the mandated follow-up testing process, a critical compliance requirement under federal aviation regulations. The FAA said the lapses occurred during multiple periods between August 2021 and July 2024, during which the employees continued performing operational roles such as flying aircraft, serving passengers, and maintaining equipment. Under U.S. aviation rules, employees who test positive must undergo a structured return-to-duty process, followed by unannounced follow-up testing for up to five years. The FAA alleges that Southwest did not fully comply with these requirements. The case highlights broader regulatory concerns around compliance oversight in safety-critical roles. FAA regulations impose strict limits, including a maximum blood alcohol level of 0.04% for pilots and mandatory waiting periods before duty, reflecting the high safety stakes in commercial aviation. Failure to enforce follow-up testing protocols undermines the system designed to ensure that employees remain fit for duty after substance-related violations. Southwest Airlines acknowledged the issue and stated that it takes its drug and alcohol testing responsibilities seriously, adding that it had already implemented enhanced procedures and stronger oversight measures more than two years ago. “We took immediate action more than two years ago to enhance our procedures and to strengthen oversight and accountability,” the airline said in a statement. The airline has been given 30 days to respond to the FAA’s enforcement notice and may choose to contest the findings or negotiate a settlement before any final penalty is imposed.

The lapse was discovered during a routine night‑halt inspection by an engineer, who noticed the aircraft’s ARC had expired. The certificate is mandatory to ensure the aircraft meets maintenance and safety standards before any flight. DGCA officials confirmed the aircraft had flown eight “revenue sectors” on 24–25 November 2025, despite the absence of a valid ARC. Flying without this certificate is considered a serious compliance violation as it directly pertains to flight safety. Following the discovery, DGCA immediately grounded the aircraft and suspended all personnel responsible for approving its release to service pending the outcome of the investigation. The regulator has also instructed Air India to conduct an internal compliance audit to identify systemic gaps and prevent future lapses. In a statement, Air India acknowledged the incident, calling it “regrettable.” The airline said it self-reported the breach to DGCA, suspended the staff involved, and initiated its own internal investigation. It reaffirmed its commitment to strict safety and regulatory compliance. Operating an aircraft without a valid airworthiness certificate poses significant safety risks and could potentially impact insurance coverage for the aircraft, including hull, liability, and passenger insurance. It has been described as a Level‑1 violation, the most serious type of compliance failure concerning aircraft safety standards. The aircraft was part of the former Vistara fleet, which merged with Air India in late 2024. After the merger, DGCA renewed ARCs for 69 of 70 aircraft. The 70th aircraft, which later operated eight sectors, had undergone an engine change, during which its ARC expired. Despite this, it was mistakenly released into service. DGCA’s inquiry will determine whether penalties, fines, or further actions against responsible personnel are warranted. Air India’s internal investigation will also examine systemic compliance gaps and recommend measures to prevent recurrence.

The DGCA’s findings point to irregularities in Akasa Air’s flight safety systems, crew duty-time compliance, and internal safety audits. The regulator reportedly observed repetitive procedural lapses and documentation gaps, including delayed closure of internal audit findings and incomplete reviews of cockpit voice and flight data recorder (CVR/FDR) analyses. The regulator also highlighted discrepancies in the alignment of Akasa Air’s Safety Management System (SMS) manual with departmental manuals governing flight operations and engineering. In addition, not all employees had completed mandatory fatigue management training, which forms a critical component of the DGCA’s flight safety oversight framework. Akasa Air, a Mumbai-based Indian airline that launched commercial operations in August 2022. It was founded by Vinay Dube and Aditya Ghosh, with substantial backing from renowned investor Rakesh Jhunjhunwala. The airline began operations on August 7, 2022, with its first flight between Mumbai and Ahmedabad, using a Boeing 737 MAX 8 aircraft. New Delhi, India: The Directorate General of Civil Aviation (DGCA) has imposed a fine of ₹20 lakh on IndiGo, India’s largest airline, for alleged lapses in its pilot training programme. The regulator found that the airline had used unqualified flight simulators to train pilots operating flights to Category C aerodromes, which are considered challenging due to terrain and approach conditions. According to the filing, the alleged non-compliance relates to the airline’s “failure to use qualified simulators for pilot training at Category C aerodromes.” These airports include Calicut, Mangaluru, Srinagar, Port Blair, Leh, Aizawl, Shimla, and international locations such as Kabul and Kathmandu. Aviation experts said that Category C airports require pilots to undergo specific simulator training replicating the topography and terrain of those airfields. “A qualified simulator must realistically depict the local geography, altitude, and approach pattern. Using a generic simulator can affect pilot readiness for critical airports like Leh or Calicut,” said Captain Anil S. Rao, President of the All India Pilots Association, in comments to The New Indian Express. In its statement, IndiGo said the fine would have no material impact on its operations and that it would contest the DGCA order before the appropriate appellate authority. “We remain committed to maintaining the highest training standards and operational safety,” the airline said. The DGCA’s order, dated September 26, 2025, stated that IndiGo failed to comply with simulator certification standards while conducting recurrent training for pilots assigned to operate at high-risk airports. The penalty was disclosed by IndiGo’s parent company, InterGlobe Aviation, in its filing to the Bombay Stock Exchange (BSE). The DGCA had earlier issued a show-cause notice to IndiGo on August 11, 2025, for using unapproved Airbus A320 Full Flight Simulators in pilot training. These simulators were reportedly used for critical training functions such as type rating, recurrent training, and proficiency checks without securing the necessary regulatory approval. The DGCA gave IndiGo seven days to respond, warning that failure to do so could have led to enforcement actions, including potential suspension of related training approvals. IndiGo acknowledged the notice and temporarily withdrew the identified simulators pending regulatory clearance. The airline cooperated with the DGCA to ensure all training devices met the required standards.
