New Delhi, India: The Federation of Indian Pilots (FIP) has intensified its challenge to the emerging narrative surrounding the Air India Flight 171 crash investigation, arguing that critical technical questions remain unanswered and urging investigators to conduct additional simulator testing before issuing a final report. Addressing a press conference on June 20, FIP President Captain C. S. Randhawa disputed suggestions that the Boeing 787's fuel control switches were deliberately moved by the flight crew, stating that “the pilots have actually not moved the switches.” The remarks come amid growing scrutiny of the Aircraft Accident Investigation Bureau's (AAIB) preliminary findings into the June 12, 2025 crash of Air India Flight 171, which killed 260 people shortly after departure from Ahmedabad. The AAIB's preliminary report concluded that both engine fuel control switches transitioned from RUN to CUTOFF shortly after takeoff, resulting in the loss of fuel supply to both engines. Cockpit voice recordings reportedly captured one pilot asking the other why the fuel had been cut off, with the second pilot denying responsibility. According to Randhawa, the report failed to include cockpit warning sequences that would normally accompany a significant electrical malfunction. Randhawa also questioned why investigators allegedly did not give sufficient weight to the testimony of the crash's sole survivor, Vishwash Kumar Ramesh. According to Randhawa, Ramesh reported seeing lights flickering inside the cabin shortly after the aircraft became airborne. The FIP president argued that this observation could be a significant clue pointing toward a possible electrical malfunction, noting that cabin lights are normally configured for takeoff and should not have been flickering under normal conditions. He said the survivor's account, when considered alongside technical material submitted by the federation regarding electrical-system anomalies and transmitted fault codes, warranted a deeper examination of a potential electrical-failure scenario rather than an exclusive focus on pilot actions. Randhawa maintained that the survivor's statement could represent an important piece of evidence in understanding the chain of events that led to the crash. FIP further highlighted Aircraft Communications Addressing and Reporting System (ACARS) maintenance messages transmitted around the time of departure. The federation has requested that Air India provide the original ACARS records and that Boeing assist investigators in decoding the data to determine whether any technical abnormalities were present before the accident. Another area of concern involves the Full Authority Digital Engine Control (FADEC) system. The impact occurred only a few hundred feet from the aircraft's final recorded position and triggered a post-crash fire that later spread into a forest fire. The preliminary report also revealed that pilots had been provided notices warning of GPS interference before departure. The flight briefing package contained a NOTAM advising that GPS, WAAS, GBAS and ADS-B services could be unavailable across a large area of New Mexico due to military operations. Additional notices indicated that Sierra Blanca Regional Airport's automated weather observation system was out of service. Investigators noted that no METAR or TAF weather reports were available for the airport at the time of the accident. The NTSB emphasized that the report is preliminary and that no probable cause has yet been determined. Investigators continue to examine the role of GPS interference, flight crew actions, navigation challenges, terrain clearance and other operational factors. Passenger Smartphone Fire Forces British Airways Flight Back To Parking Stand In Tokyo

Paringa, New Zealand: A loose maintenance bolt that detached during flight caused an Airbus AS350BA helicopter to become increasingly difficult to control before it crashed near the Paringa River mouth on New Zealand's West Coast, according to final findings released by the Transport Accident Investigation Commission (TAIC). The accident occurred on July 20, 2024, while an Amuri Helicopters Airbus AS350BA, registration ZK-HJM, was being flown from Queenstown to Franz Josef following maintenance work performed at Queenstown Aerodrome. The helicopter was carrying a pilot and one passenger. Both occupants sustained minor injuries and were transported to hospital, while the aircraft suffered substantial damage. According to the investigation, engineers from Salus Aviation had swapped the helicopter's left and right hydraulic servo actuators during scheduled maintenance. However, a critical securing bolt on the lower attachment point of the left-hand hydraulic servo actuator was not properly secured before the aircraft was returned to service. Investigators determined that the associated nut had not been tightened to the required torque and that a locking pin was either missing or incorrectly installed. Although the work was subject to a mandatory duplicate inspection by a second engineer, the defect went undetected. Approximately 52 minutes into the flight, the pilot felt a sudden jolt and observed abnormal main rotor blade tracking. Seven minutes later, concerned by the abnormal indications, the pilot initiated a precautionary landing. During the descent, cyclic control forces increased significantly, particularly when attempting left control inputs. As the helicopter descended to between three and seven metres above the ground, it rolled and collided with terrain before coming to rest on its left side. TAIC found that the securing bolt eventually detached completely during the flight, significantly limiting the pilot's control of the helicopter. The investigation identified several human-factor issues that contributed to the maintenance error. The engineer responsible for the task was reportedly distracted by both a customer interaction and a hydraulic fluid spill while performing the work. Investigators also found the engineer was working under time pressure ahead of a planned holiday and did not sufficiently verify the maintenance task before completion. The second engineer responsible for the duplicate safety inspection also failed to identify the improperly secured hardware. The engineer told investigators they could not explain why the defect was missed but acknowledged experiencing broken sleep at the time, which may have affected performance. Following the accident, Salus Aviation conducted an internal investigation and implemented a series of corrective actions. The company temporarily removed the involved engineers from release-to-service inspection duties, introduced additional training, strengthened inspection and documentation procedures, implemented independent post-maintenance inspections, and established regular assurance audits. Measures aimed at reducing workplace distractions and improving engineering oversight were also introduced. TAIC concluded that the maintenance provider had adequately addressed the safety deficiencies identified during the investigation. As a result, the Commission did not issue any formal safety recommendations.

Loose Bolt Left Undetected During Inspection Caused New Zealand Airbus AS350 Crash

Garden Grove, United States: More than 30 lawsuits have been filed against GKN Aerospace following a chemical tank emergency at the company's Garden Grove, California, facility that forced the evacuation of approximately 50,000 residents and triggered multiple state and federal investigations. The legal actions stem from an incident that began on May 21, when a storage tank containing between 6,000 and 7,000 gallons of methyl methacrylate (MMA), a highly flammable chemical used in acrylic plastics and aerospace manufacturing, began overheating at GKN Aerospace's Transparency Systems facility in Orange County. Authorities feared the tank could rupture, ignite, or trigger a catastrophic explosion capable of endangering surrounding communities and nearby chemical storage tanks. The emergency prompted widespread evacuation orders across portions of Garden Grove and neighboring communities, including Anaheim, Cypress, Stanton, Westminster and Buena Park, during the Memorial Day holiday period. According to emergency officials, the tank's internal temperature rose steadily as responders worked around the clock to cool and stabilize the vessel. On June 10, agents from the Federal Bureau of Investigation and the U.S. Environmental Protection Agency executed a federal search warrant at the Garden Grove facility. Investigators collected documents, electronic records, maintenance files, inspection records and chemical samples as part of an inquiry into the circumstances surrounding the tank failure. Federal officials said investigators are examining whether any federal environmental or industrial safety laws were violated. The warrant authorized the seizure of records related to the storage, handling, temperature control, maintenance, use and disposal of methyl methacrylate, as well as communications among company personnel regarding the chemical and storage systems. Federal authorities also brought EPA scientists and chemists to the site to collect evidence, conduct testing and assist investigators. Officials have indicated that the inquiry remains active and could take months to complete. In parallel, the Orange County District Attorney's Office has launched a separate criminal investigation into the incident. The emergency also renewed scrutiny of regulatory oversight surrounding the facility. Reports have highlighted previous regulatory actions involving the site, including workplace safety citations and a 2025 air-quality settlement with the South Coast Air Quality Management District. Community leaders and residents have questioned whether hazardous chemical storage should continue in close proximity to densely populated residential neighborhoods. During public meetings held after the evacuation orders were lifted, residents urged GKN Aerospace to relocate its chemical storage operations or implement stronger safety protections. Local officials have also criticized the company's initial financial assistance package. GKN Aerospace announced community assistance totaling several million dollars to help offset emergency costs incurred by local governments and residents. However, some elected officials described the contribution as insufficient given the scale of the disruption and emergency response. 30+ Lawsuits Target GKN Aerospace After Chemical Tank Incident Forced 50,000 Evacuations

Rio de Janeiro, Brazil: The International Air Transport Association (IATA) has highlighted Brazil’s significant potential to become a global leader in Sustainable Aviation Fuel (SAF) production, positioning the country as a key contributor to aviation’s decarbonisation efforts while strengthening energy security and driving economic growth. The assessment was presented during IATA’s 82nd Annual General Meeting in Rio de Janeiro, where the association outlined Brazil’s strategic advantages in supporting the aviation industry's transition toward net-zero carbon emissions by 2050. According to IATA, airlines worldwide will require approximately 500 million tonnes (Mt) of SAF annually by 2050 to meet the sector’s net-zero commitment. Brazil is well positioned to support this demand due to its extensive biomass resources and established biofuels industry. IATA estimates that Brazil possesses one of the world's largest biomass feedstock potentials, capable of reaching around 180 million tonnes by 2050, which could support the production of approximately 60 million tonnes of SAF. In the near term, sustainably sourced sugarcane ethanol along with virgin and waste oil feedstocks could provide around 18 million tonnes of feedstock by 2030, translating into approximately 12 million tonnes of SAF production potential. IATA noted that this volume would be roughly five times greater than the estimated global SAF production of 2.4 million tonnes expected in 2026. Brazil currently has around 15 SAF development projects underway. If all projects proceed as planned, they could collectively bring approximately 2 million tonnes of SAF production capacity online. “Brazil has all the ingredients to be a global SAF powerhouse. It has one of the cleanest electricity mixes in the world as well as abundant feedstock. Moreover, as the second largest producer of liquid biofuels in the world, the country benefits from deep expertise and developed infrastructure. Brazil has a real opportunity to be a global leader in aviation’s decarbonisation. Embracing this opportunity will create jobs, reduce dependence on foreign fossil fuels, build new energy and agriculture industries, and grow the economy. With the right policies implemented in the right order, Brazil is ready to jump-start the market,” said Willie Walsh, IATA Director General. Beyond feedstock availability, IATA emphasized that Brazil’s extensive experience in ethanol production and its established refining infrastructure provide substantial competitive advantages for SAF development. These strengths create a solid foundation for scaling production through Hydroprocessed Esters and Fatty Acids (HEFA) pathways and other advanced SAF technologies, particularly ethanol-to-jet fuel processes. The association believes this could eventually position Brazil as a net exporter of sustainable aviation fuel. IATA further noted that large-scale SAF production could generate substantial economic benefits across the entire value chain, including agriculture, feedstock cultivation, logistics, infrastructure development, refining, advanced fuel manufacturing and export activities. The association said such growth would support job creation, enhance energy independence and security, improve soil quality, strengthen natural capital and deliver benefits to local communities. Despite the country's significant potential, IATA acknowledged that SAF production in Brazil remains at an early stage and that achieving industrial-scale output will require several critical enablers. Among the priorities identified are investments in conversion technologies, expanded infrastructure and improved logistics networks to efficiently connect feedstock suppliers with fuel production facilities. IATA also called for targeted policy incentives designed to stimulate production, strengthen financing mechanisms and ensure alignment with internationally recognized sustainability standards. Another important element highlighted by the association is the implementation of a book-and-claim system based on tradable SAF certificates. IATA welcomed plans for Brazil’s Fuel of the Future framework (ProBioQAV) to incorporate such mechanisms alongside requirements for airline SAF usage. According to IATA, the approach would facilitate integration with international programs such as the Carbon Offsetting and Reduction Scheme for International Aviation (CORSIA) while helping create a functioning SAF market. The association stressed that policy sequencing will be crucial, arguing that adequate SAF supply must be established before usage mandates take effect. “Brazil has many advantages both in terms of natural resources and vast experience that should give the country a world-leading role in SAF markets. Announced on June 16 by the Department for Transport, the funding programme will provide targeted support to companies developing low-carbon fuels, with £93 million set to be made available during the next two years. Tennessee, United States: Two brothers were killed when a small aircraft crashed in Warren County, Tennessee, on June 17, prompting a federal investigation by the National Transportation Safety Board (NTSB) and the Federal Aviation Administration (FAA). According to Warren County Sheriff Jackie Matheny Jr., the aircraft departed from a private airstrip in the Viola community before crashing in a cornfield near Morrison Industrial Park, west of McMinnville. The crash site was located near Doyle Frazier Road in Morrison. Preliminary information released by the sheriff's office indicates investigators believe the aircraft may have struck a tree shortly after takeoff. Despite the apparent impact, the airplane continued flying for a short period before ultimately crashing while likely en route to Warren County Memorial Airport. Sheriff Matheny said the aircraft appeared to have entered a nose-down descent before impact, leaving behind a large debris field across the crash site. Family members confirmed that the victims were brothers from the Viola community. The aircraft involved was a Cessna 177RG Cardinal. Authorities confirmed that both occupants died in the accident. Emergency crews responded to the scene shortly after the crash was reported at approximately 12:19 p.m. local time. The accident occurred in a rural area near Morrison Industrial Park, and no injuries on the ground were reported. Federal investigators have now assumed responsibility for determining the cause of the accident. The NTSB is leading the investigation with assistance from the FAA. Investigators are expected to examine the wreckage, aircraft records, pilot information, weather conditions, and other factors as part of the inquiry. Automation Rises, But Are Indian Pilots Being Trained To Keep Pace With The Future?

START: India is emerging as the fastest-growing aviation market worldwide, yet it faces a significant hurdle: a shortage of trained technical personnel, including pilots, engineers, and technicians. This challenge is expected to become more acute as Electric Vertical Takeoff and Landing (eVTOL) vehicles enter the market within the next year or two, creating an additional need for pilots. The issue stems largely from inadequate planning and foresight by the authorities. Consequently, many aspiring Indian pilots are pursuing training in countries like Sri Lanka, Thailand, Bosnia, Montenegro, and the Philippines. In fact, approximately 700 out of every 1,000 commercial pilot licenses issued by the Directorate General of Civil Aviation (DGCA) are earned abroad. India’s Potential as a Global Hub for Aviation Education Considering India’s expansive geography and favorable weather for flying, the country is ideally suited to be a global hub for aviation education. In the 1980s and 1990s, students from nations such as Iran, Jordan, Palestine, Afghanistan, Sri Lanka, Bhutan, and several African countries came to India for flight training. Today, the trend has reversed, with Indian students going overseas for pilot education. One key reason is the slow pace of domestic training programs and the cumbersome security clearance process for foreign students, which discourages them from enrolling in India. Competition from International Flight Schools Meanwhile, nations like the United Arab Emirates, Qatar, and Saudi Arabia have established world-class flight schools. Students from neighboring Arab countries now mostly prefer the United States, Canada, and Europe for training, despite these regions lacking ideal weather for primary flight instruction. Domestic regulations further complicate matters. The DGCA allows a flight school to operate only if it meets minimum requirements for aircraft and paid-up capital, while the Airports Authority of India (AAI) imposes additional capital requirements. These rules do not guarantee quality education but are often sufficient to prevent many capable entrepreneurs from opening flight schools. By contrast, an Indian entrepreneur started a flight school in New Zealand with a single rented aircraft, which has since expanded to a fleet of over 20 planes and now attracts Indian students. Shortage of Trainer Aircraft The shortage of trainer aircraft is another pressing challenge. In the 1970s, Prof. David Hazen of Princeton University, invited by the Indian government to establish the first aeronautical department at IIT Kanpur, outlined a roadmap for India to achieve self-reliance in aviation training. However, progress has been slow. The DGCA developed the Swati light aircraft, manufactured by Bharat Heavy Electricals Limited (BHEL), but a fatal accident during testing brought the program to an end. Similarly, the Hansa trainer created by the National Aerospace Laboratory (NAL) was rejected by most flight schools. Lessons from the Pushpak Aircraft A comparison with the Pushpak aircraft is instructive. Manufactured in the 1950s, HAL produced 160 Pushpaks, which served India reliably for over 50 years and maintained an excellent safety record. In 1968, the government gifted one Pushpak each to Singapore and Malaysia to help them establish their flight training programs. The Pushpak’s success was largely due to it being modeled closely on the proven US Aeronca Chief. NAL later developed the next-generation Hansa (NG Hansa), which was showcased at the 2020 Wings India airshow, but production has yet to start, and setting up the necessary infrastructure would still take several years even if action begins immediately. Global Trainer Aircraft Shortages and LSAs The shortage of trainer aircraft is not unique to India. Major manufacturers such as Cessna and Piper had slowed production, underestimating demand, and now struggle to meet global requirements. Other countries have tackled this problem by incorporating Light Sport Aircraft (LSAs) into flight training. Equipped with advanced navigation systems, LSAs are cost-effective, environmentally friendly, and widely used in the US and EU for commercial pilot instruction. However, Indian regulations currently do not permit LSAs for commercial pilot training. Despite being equipped with only a magnetic compass, pilots trained on Pushpaks have gone on to fly advanced aircraft like the Boeing 777 and Airbus A320. Allowing LSAs in India could reduce aircraft shortages, encourage domestic manufacturing, generate employment, and attract foreign investment. Lack of Experienced Flight Instructors The lack of experienced flight instructors adds another layer of difficulty. Rapid growth in airlines such as Indigo and Air India has led many instructors to leave training schools for better pay, career progression, and recognition in commercial aviation. This high turnover leaves less-experienced instructors teaching students, unlike other professional fields where teachers usually have extensive industry experience. In countries like the US, airline pilots can provide primary flight training in their spare time, giving trainees the advantage of learning from seasoned professionals and ensuring higher safety standards.
