Paringa, New Zealand: A loose maintenance bolt that detached during flight caused an Airbus AS350BA helicopter to become increasingly difficult to control before it crashed near the Paringa River mouth on New Zealand's West Coast, according to final findings released by the Transport Accident Investigation Commission (TAIC). The accident occurred on July 20, 2024, while an Amuri Helicopters Airbus AS350BA, registration ZK-HJM, was being flown from Queenstown to Franz Josef following maintenance work performed at Queenstown Aerodrome. The helicopter was carrying a pilot and one passenger. Both occupants sustained minor injuries and were transported to hospital, while the aircraft suffered substantial damage. According to the investigation, engineers from Salus Aviation had swapped the helicopter's left and right hydraulic servo actuators during scheduled maintenance. However, a critical securing bolt on the lower attachment point of the left-hand hydraulic servo actuator was not properly secured before the aircraft was returned to service. Investigators determined that the associated nut had not been tightened to the required torque and that a locking pin was either missing or incorrectly installed. Although the work was subject to a mandatory duplicate inspection by a second engineer, the defect went undetected. Approximately 52 minutes into the flight, the pilot felt a sudden jolt and observed abnormal main rotor blade tracking. Seven minutes later, concerned by the abnormal indications, the pilot initiated a precautionary landing. During the descent, cyclic control forces increased significantly, particularly when attempting left control inputs. As the helicopter descended to between three and seven metres above the ground, it rolled and collided with terrain before coming to rest on its left side. TAIC found that the securing bolt eventually detached completely during the flight, significantly limiting the pilot's control of the helicopter. The investigation identified several human-factor issues that contributed to the maintenance error. The engineer responsible for the task was reportedly distracted by both a customer interaction and a hydraulic fluid spill while performing the work. Investigators also found the engineer was working under time pressure ahead of a planned holiday and did not sufficiently verify the maintenance task before completion. The second engineer responsible for the duplicate safety inspection also failed to identify the improperly secured hardware. The engineer told investigators they could not explain why the defect was missed but acknowledged experiencing broken sleep at the time, which may have affected performance. Following the accident, Salus Aviation conducted an internal investigation and implemented a series of corrective actions. The company temporarily removed the involved engineers from release-to-service inspection duties, introduced additional training, strengthened inspection and documentation procedures, implemented independent post-maintenance inspections, and established regular assurance audits. Measures aimed at reducing workplace distractions and improving engineering oversight were also introduced. TAIC concluded that the maintenance provider had adequately addressed the safety deficiencies identified during the investigation. As a result, the Commission did not issue any formal safety recommendations.

The incident occurred on 1 December 2024, when the aircraft was en route from Wellington to Sydney. Approximately 40 minutes after takeoff, the No. 2 engine unexpectedly shut down following activation of the overhead fire‑switch despite no pilot input. The crew declared a mayday and safely diverted to Auckland Airport, with all 154 passengers and crew unharmed. TAIC’s report identifies internal damage to the fire‑switch mechanism as the primary cause. A retaining pin inside the switch had bent, reducing the latching security and allowing vibration or normal operation to trigger the switch inadvertently. Inspections revealed that the panel surrounding the switch also showed signs of deformation, further contributing to the uncommanded activation. “The fire‑switch is designed to be manually operated only in case of an actual engine fire,” TAIC noted. “Its activation without pilot input posed a significant safety risk.” The commission also highlighted that the same fire‑panel design is used across the global A320 fleet indicating that other aircraft could potentially be affected. The switch manufacturer had issued service bulletins in late 2024 recommending inspection of previously repaired panels, but those checks focused on visually detectable external damage. Following the ANZ incident, Airbus expanded its guidance to inspect all affected fire‑panels for any signs of deformation within 1,000 flight cycles. The European Union Aviation Safety Agency (EASA) has since issued an Airworthiness Directive mandating such inspections and, where necessary, replacement of defective panels. Air New Zealand reportedly removed suspect panels from its fleet in early December 2024. On 28 November 2025, Airbus triggered another major safety action ordering a global recall of approximately 6,000 A320‑family aircraft, following a serious mid‑air flight‑control issue linked to corruption of flight‑control data under intense solar radiation. In a statement, Airbus described the recall as “precautionary,” urging all operators worldwide to apply an immediate software rollback (or, where required, hardware modifications) before affected aircraft are flown again. This recall reportedly the largest in Airbus’s 55‑year history affects over half of the global A320 fleet and has already prompted airlines across the U.S., Europe, Asia and beyond to ground jets for urgent software updates.

Flight AF652, operated by a Boeing 777-328(ER) registered F-GZNG, took off from CDG at 18:01 local time en route to Saint-Denis, Réunion. About 50 nautical miles southeast of Zurich, the flight crew reported rapidly rising cabin temperatures and an unusual smell of overheating. Passengers reportedly described the cabin as turning into a “sauna,” with the temperature climbing to uncomfortable levels within minutes. According to Business Insider, the crew first issued a “PAN-PAN” urgent call before upgrading to a “MAYDAY” to request priority handling and clearance to dump fuel over French territory prior to returning to Paris. The aircraft landed safely at Charles de Gaulle approximately two hours after departure. Air France confirmed the incident, stating that the flight “experienced a technical malfunction in the cabin temperature regulation system,” and that the decision to return was made “in accordance with standard safety procedures.” All 312 passengers and 14 crew members disembarked safely, and no injuries were reported. A replacement aircraft was dispatched later that evening, completing the journey to Réunion with a delay of about six hours. An initial review indicates a fault in the Environmental Control System (ECS), which regulates temperature and air circulation. The specific cause whether a software issue, valve malfunction, or sensor failure has not yet been confirmed. France’s Bureau d’Enquêtes et d’Analyses (BEA) is expected to open an investigation into the technical sequence that led to the overheating. Air France issued an apology to passengers, reaffirming that “safety remains the airline’s top priority.”
