Investigators noted that the defective valve on the SWISS aircraft remained in service far beyond its recommended inspection and replacement interval, even after being removed and inspected multiple times without upgrading to the redesigned part. The report concluded that this prolonged delay in corrective maintenance directly contributed to the in-flight incident. In its final recommendations, the STSB urged the European Union Aviation Safety Agency (EASA) to mandate the skin check valve replacement on all applicable Airbus A330 and A340 aircraft via a formal Airworthiness Directive rather than relying on voluntary compliance with service bulletins. The board cautioned that gradual decompression events can elude crew awareness in early stages, increasing risk if they occur at cruising altitudes. As per its FY25 annual report, IndiGo had 5,456 pilots in total, including captains and first officers. India’s civil-aviation regulator, DGCA, has granted a one-time relaxation of certain night-duty and crew-rest provisions under the revised Flight Duty Time Limitation (FDTL) norms to help stabilize operations after hundreds of flight cancellations last week by IndiGo. The relaxation, valid until 10 February 2026, covers night-flying limits and allows leave to count as part of weekly rest periods. The airline’s heavy reliance on high-utilisation A320 operations means that each additional flying hour amplifies its pilot requirement. The combined impact of delays and cancellations caused widespread disruption for domestic travellers, leaving passengers stranded, scrambling for alternatives, and highlighting the operational strain on the airline’s network. DGCA officials confirmed the aircraft had flown eight “revenue sectors” on 24–25 November 2025, despite the absence of a valid ARC. Flying without this certificate is considered a serious compliance violation as it directly pertains to flight safety. Following the discovery, DGCA immediately grounded the aircraft and suspended all personnel responsible for approving its release to service pending the outcome of the investigation. According to officials, the incident took place during the flight on Friday, November 28, when the accused passenger, reportedly under the influence of alcohol, allegedly touched the cabin crew member inappropriately during the journey. The flight landed at Rajiv Gandhi International Airport (RGIA), Hyderabad, where security agencies were alerted immediately after the complaint was received. Police said that after the aircraft landed, crew members discovered a handwritten note containing obscene and abusive remarks left behind on the passenger’s seat. The matter was escalated to airport security and local law enforcement, following which the accused was taken into custody for questioning. A formal case was registered under relevant provisions of the Bharatiya Nyaya Sanhita (BNS), including sections related to sexual harassment and use of criminal force against a woman with intent to outrage her modesty. The accused was produced before a local court and has been remanded to judicial custody, police added. Investigators are also examining whether the passenger consumed alcohol before boarding or during the flight. Officials stated that strict action would be taken based on the findings of the probe and supporting evidence provided by the airline and crew members. The airline has so far not issued a detailed official statement on the matter. However, aviation authorities reiterated that any form of misconduct toward airline staff or fellow passengers will attract strict legal consequences.
