Washington, United States: The Federal Aviation Administration (FAA) has issued an Emergency Airworthiness Directive targeting all Embraer Model EMB-545 and EMB-550 aircraft, citing a potentially catastrophic failure risk involving the aircraft’s pitch trim system. The directive, which became effective immediately upon receipt, follows reports of in-service failures in one load path of the pitch trim actuator, a critical component that controls the horizontal stabilizer. According to the FAA, such failures, if unaddressed, could escalate into a dual load-path failure, potentially allowing the stabilizer to move freely under aerodynamic forces and resulting in loss of control of the aircraft. The action stems from an earlier emergency directive issued by Brazil’s aviation authority, the Agência Nacional de Aviação Civil (ANAC), under Emergency AD 2026-04-02, which identified the unsafe condition during scheduled maintenance checks. Specifically, failures were observed during operational checks of pitch trim actuator irreversibility, raising concerns over the system’s redundancy. Under bilateral aviation safety agreements, ANAC notified the FAA of the issue, prompting the U.S. regulator to determine that the same unsafe condition is likely to exist or develop in aircraft operating under U.S. jurisdiction. The FAA directive mandates operators to carry out operational checks of the pitch trim actuator, with an option to perform a pitch trim verification, in accordance with the ANAC procedures. If the system fails, defined as a “TEST FAILED” status after five minutes or repeated “TEST ABORTED” results after five attempts operators must replace the actuator before further flight. Additionally, if a replacement is required, a subsequent operational check must be performed on the new actuator to ensure airworthiness before the aircraft returns to service. The directive also requires operators to report inspection and verification data within 10 days, either after completing the checks or from the date of receiving the directive, depending on when the inspection was conducted. Citing an urgent safety risk, the FAA bypassed the standard notice-and-comment rulemaking process under the Administrative Procedure Act, invoking “good cause” to enforce the directive immediately. The agency stated that the short compliance window and the severity of the potential failure made prior public consultation impractical and contrary to the public interest. “The risk to the flying public justifies forgoing notice and comment,” the FAA noted, emphasizing that unresolved actuator failures could lead to uncontrolled stabilizer movement and loss of aircraft control. The FAA described the directive as an interim action, noting that the required reporting will help manufacturers better understand the root cause, scope, and frequency of the failures. Based on the findings, further rulemaking or permanent corrective measures may follow. The directive applies to all EMB-545 and EMB-550 aircraft, regardless of certification category, and falls under ATA Chapter 27 (Flight Controls). No previous airworthiness directives are superseded by this action. The FAA issued the directive under its statutory authority outlined in Title 49 of the United States Code, which mandates the agency to ensure safe flight operations by prescribing necessary regulations and procedures.

The regulator has warned the airline that failure to respond fully within one week could trigger regulatory action affecting its Dreamliner operations. Air India flight AI132, operating a Boeing 787-8 Dreamliner registered VT-ANX, departed Heathrow for Bengaluru after pilots noted irregular behaviour with a fuel control switch, a vital cockpit control that regulates fuel flow to the engines. During engine start, one of the two fuel control switches did not remain positively latched in the “RUN” position on the first two attempts before stabilising on the third. The flight continued without in-air issues and landed in Bengaluru. Upon arrival, the crew filed a defect report citing the anomaly, and the aircraft was grounded for inspection on February 2. The UK Civil Aviation Authority has taken the step of demanding a formal response from Air India, including: A comprehensive account of pre-flight maintenance actions taken before departure, A root-cause analysis of the fuel control switch event, A preventive action plan to reduce the risk of recurrence across Air India’s Dreamliner fleet, which comprises 33 aircraft. In official correspondence, the CAA emphasised that regulators routinely seek detailed explanations following incidents affecting safety-critical components, particularly when an aircraft departs with a potential defect. Air India said it conducted precautionary re-inspections of fuel control switches across its Boeing 787 fleet after the incident and reported no issues found during these checks. The airline acknowledged the Directorate General of Civil Aviation (DGCA) oversight and said it will comply with the regulator’s guidance to circulate Boeing’s recommended operating procedures for the switches to all flight crews. India’s DGCA also issued a clarification, stating that post-flight inspections determined the fuel switches were serviceable and functioning normally. According to the regulator, the switch may move if light pressure is applied in an incorrect direction, but its design does not indicate a defect when used according to approved procedures. The DGCA advised Air India to reinforce correct switch handling among pilots. Fuel control switches are crucial for aircraft safety. They allow pilots to start and shut down engines and are designed with locking mechanisms to prevent unintended movement. This category of component has been under heightened scrutiny following the June 2025 crash of an Air India Boeing 787-8 in Ahmedabad, where preliminary investigation reports indicated fuel control switches played a role in dual-engine power loss shortly after takeoff. That accident, which resulted in significant loss of life, triggered industry-wide inspections and close regulatory observation of Boeing 787 systems. Though the final accident report remains pending, aviation authorities worldwide have been monitoring similar technical aspects, including fuel system controls, in current operations. Air India faces a deadline to submit its response to the UK CAA within one week of the February 4 notice. Vancouver, Canada: Canadian aviation authorities have launched a formal safety probe after an Air India pilot was found to be under the influence of alcohol before operating an international flight from Vancouver, prompting an inquiry and corrective action demands to the Indian carrier. The incident occurred on December 23, 2025, at Vancouver International Airport ahead of Flight AI186, scheduled to operate from Vancouver to Delhi via Vienna. According to a letter from Transport Canada Civil Aviation (TCCA) to Air India officials, the Royal Canadian Mounted Police (RCMP) reported that the pilot, identified as Captain Saurabh Kumar, reported for duty while under the influence of alcohol and was deemed unfit for the flight. Canadian authorities conducted two breathalyser tests that confirmed the presence of alcohol, following which the pilot was advised to leave the aircraft and was subsequently removed from duty at the scene. Transport Canada has characterised the occurrence as a violation of Canadian Aviation Regulations (CARs), citing breaches of sections 602.02 and 602.03, which govern crew fitness for duty and alcohol consumption limits, as well as conditions of Air India’s Foreign Air Operator Certificate (FAOC). In its correspondence, Transport Canada has formally asked Air India to investigate the matter under its Safety Management System and submit a detailed corrective action report by January 26, 2026. The regulator indicated that enforcement action by both the RCMP and the TCCA is likely as the review progresses. Air India confirmed that Flight AI186 experienced a last-minute delay on December 23 after the cockpit crew member was offloaded prior to departure “based on concerns raised by the Canadian authorities regarding the pilot’s fitness for duty.” The airline said an alternate pilot was rostered in accordance with safety protocols, resulting in the delayed departure. In an official statement, Air India said it regrets the inconvenience caused to passengers, is fully cooperating with Canadian authorities, and has taken the pilot off flying duties pending the outcome of the inquiry. The airline reiterated a zero-tolerance policy toward violations of applicable rules and regulations, noting that “any confirmed violation will attract strict disciplinary action in line with company policy.” Initial accounts of the incident suggest that the episode first came to light after a duty-free shop employee at the Vancouver airport reported a concern about the pilot either smelling of alcohol or consuming it moments before boarding preparations. This triggered intervention by Canadian authorities and the subsequent breathalyser tests.
