Vancouver, Canada: Canadian aviation authorities have launched a formal safety probe after an Air India pilot was found to be under the influence of alcohol before operating an international flight from Vancouver, prompting an inquiry and corrective action demands to the Indian carrier. The incident occurred on December 23, 2025, at Vancouver International Airport ahead of Flight AI186, scheduled to operate from Vancouver to Delhi via Vienna. According to a letter from Transport Canada Civil Aviation (TCCA) to Air India officials, the Royal Canadian Mounted Police (RCMP) reported that the pilot, identified as Captain Saurabh Kumar, reported for duty while under the influence of alcohol and was deemed unfit for the flight. Canadian authorities conducted two breathalyser tests that confirmed the presence of alcohol, following which the pilot was advised to leave the aircraft and was subsequently removed from duty at the scene. Transport Canada has characterised the occurrence as a violation of Canadian Aviation Regulations (CARs), citing breaches of sections 602.02 and 602.03, which govern crew fitness for duty and alcohol consumption limits, as well as conditions of Air India’s Foreign Air Operator Certificate (FAOC). In its correspondence, Transport Canada has formally asked Air India to investigate the matter under its Safety Management System and submit a detailed corrective action report by January 26, 2026. The regulator indicated that enforcement action by both the RCMP and the TCCA is likely as the review progresses. Air India confirmed that Flight AI186 experienced a last-minute delay on December 23 after the cockpit crew member was offloaded prior to departure “based on concerns raised by the Canadian authorities regarding the pilot’s fitness for duty.” The airline said an alternate pilot was rostered in accordance with safety protocols, resulting in the delayed departure. In an official statement, Air India said it regrets the inconvenience caused to passengers, is fully cooperating with Canadian authorities, and has taken the pilot off flying duties pending the outcome of the inquiry. The airline reiterated a zero-tolerance policy toward violations of applicable rules and regulations, noting that “any confirmed violation will attract strict disciplinary action in line with company policy.” Initial accounts of the incident suggest that the episode first came to light after a duty-free shop employee at the Vancouver airport reported a concern about the pilot either smelling of alcohol or consuming it moments before boarding preparations. This triggered intervention by Canadian authorities and the subsequent breathalyser tests.

In December 2022, during a severe winter storm, Southwest cancelled nearly 17,000 flights, leaving over 2 million passengers stranded across the United States. The airline’s crew-rescheduling and operations control systems collapsed under pressure. In December 2023, after a formal investigation, DOT imposed a record-breaking US$ 140 million civil penalty on the airline, the largest fine ever issued by the agency to a U.S. carrier for consumer-protection violations. The settlement included US$ 35 million payable to the U.S. Treasury, with the balance to be covered through credits and compensation to affected customers. Southwest had already made two payments of US$ 12 million each in early 2024 and early 2025. The remaining US$ 11 million was scheduled for payment by January 31, 2026. In its updated consent order, DOT cited “significantly improved on-time performance and completion factor” demonstrated by Southwest, attributing this turnaround to the airline’s substantial investments in modernizing its operations. According to DOT, these improvements justified crediting the remaining US$ 11 million rather than demanding payment. Southwest reportedly invested more than US$ 1 billion since the 2022 crisis to upgrade its internal systems, enhance crew scheduling and tracking capabilities, and overhaul its network operations. DOT said the waiver is in the “public interest,” arguing that incentivising airlines to improve reliability and resilience ultimately benefits consumers more than simply collecting a monetary penalty. In a statement, Southwest expressed gratitude, noting that the last two years saw an “operational turnaround” that delivered better on-time performance and lower cancellation rates, directly benefiting passengers.

These notices follow voluntary disclosures by Air India just days before a scheduled regulatory audit of its Delhi operations, covering incidents from 2023 and 2024. Sources reviewed by Economic Times indicate pilots in multiple instances exceeded weekly duty hour limits, including flights operated on June 24, 2024, and June 15, 2025, without the mandated rest periods. The regulator also noted that pilots who had completed simulator training failed to commence active flying within the specified timeframe, rendering the training invalid. Additionally, at least 17 pilots operated into and out of Kathmandu without mandatory high-altitude airport simulation training, despite its complex terrain and special approach requirements. Cabin crew shortages were also reported on four ultra-long-haul flights to North America in April and May, which were operated with as few as 12 cabin crew members, instead of the required 15. The DGCA has characterized these as serious procedural breaches and gave Air India two weeks to submit explanations and corrective plans. Failure to comply could lead to penalties, leadership accountability measures and further regulatory action. The regulator’s notices add to a series of safety-related actions taken against Air India in 2025, including nine show-cause notices in the first half of the year alone. These issues have gained urgency following the June 12 crash of Air India Flight AI 171 near Ahmedabad, which killed 260 people and has become India’s deadliest aviation disaster in over a decade. The accident has intensified scrutiny of Air India’s governance, safety oversight and crew management practices. In a statement, Air India confirmed receipt of the notices and stressed that all violations were voluntarily disclosed as part of its internal compliance checks. The airline said it remains committed to passenger and crew safety and is implementing measures to prevent recurrence. The DGCA has warned that repeated violations despite earlier advisories point to systemic governance failures and could result in stricter enforcement, fines and even leadership restructuring if compliance gaps remain unaddressed.
