Auckland, New Zealand: An Airbus A320‑Neo operated by Air New Zealand (Flight NZ249) experienced an uncommanded engine shutdown mid‑flight last year, due to a defective fire‑switch, the Transport Accident Investigation Commission (TAIC) confirmed in its interim report released on 4 December 2025. The incident occurred on 1 December 2024, when the aircraft was en route from Wellington to Sydney. Approximately 40 minutes after takeoff, the No. 2 engine unexpectedly shut down following activation of the overhead fire‑switch despite no pilot input. The crew declared a mayday and safely diverted to Auckland Airport, with all 154 passengers and crew unharmed. TAIC’s report identifies internal damage to the fire‑switch mechanism as the primary cause. A retaining pin inside the switch had bent, reducing the latching security and allowing vibration or normal operation to trigger the switch inadvertently. Inspections revealed that the panel surrounding the switch also showed signs of deformation, further contributing to the uncommanded activation. “The fire‑switch is designed to be manually operated only in case of an actual engine fire,” TAIC noted. “Its activation without pilot input posed a significant safety risk.” The commission also highlighted that the same fire‑panel design is used across the global A320 fleet indicating that other aircraft could potentially be affected. The switch manufacturer had issued service bulletins in late 2024 recommending inspection of previously repaired panels, but those checks focused on visually detectable external damage. Following the ANZ incident, Airbus expanded its guidance to inspect all affected fire‑panels for any signs of deformation within 1,000 flight cycles. The European Union Aviation Safety Agency (EASA) has since issued an Airworthiness Directive mandating such inspections and, where necessary, replacement of defective panels. Air New Zealand reportedly removed suspect panels from its fleet in early December 2024. On 28 November 2025, Airbus triggered another major safety action ordering a global recall of approximately 6,000 A320‑family aircraft, following a serious mid‑air flight‑control issue linked to corruption of flight‑control data under intense solar radiation. In a statement, Airbus described the recall as “precautionary,” urging all operators worldwide to apply an immediate software rollback (or, where required, hardware modifications) before affected aircraft are flown again. This recall reportedly the largest in Airbus’s 55‑year history affects over half of the global A320 fleet and has already prompted airlines across the U.S., Europe, Asia and beyond to ground jets for urgent software updates.

Flight GF 274, carrying 154 passengers, departed Bahrain at 22:20 hrs on Saturday and was scheduled to arrive at Hyderabad’s Rajiv Gandhi International Airport at 04:55 a.m. On Sunday. At around 3 a.m., an email warning of an explosive device onboard was sent to the Hyderabad airport’s customer support address, prompting authorities to initiate emergency protocols. Following standard procedure, the aircraft was instructed to divert to Chhatrapati Shivaji Maharaj International Airport, Mumbai, where it landed safely. Security teams conducted multiple layers of inspection and found no suspicious objects. After receiving clearance from security agencies, the flight departed Mumbai and landed in Hyderabad at 11:31 a.m. Gulf Air stated that the diversion was carried out “in accordance with safety and security requirements” and confirmed that all passengers were safe. Hyderabad Airport officials filed a complaint with local police, who have registered a case and begun tracing the origin of the threat email. Authorities noted that the message appeared similar to earlier hoax threats received in recent weeks. The incident follows a series of false alarms involving flights bound for Hyderabad. Earlier this month, an IndiGo flight from Jeddah to Hyderabad was diverted to Mumbai after an email referencing a past airport blast claimed a bomb was onboard. That threat also proved to be false, but it prompted full emergency procedures and inspections. New Delhi, India: The Directorate General of Civil Aviation (DGCA) has directed Air India to carry out a comprehensive inspection of the Ram Air Turbine (RAT) systems across its entire fleet of Boeing 787 Dreamliners. This action comes after an incident in which the RAT deployed automatically on one of the airline’s aircraft during landing in Birmingham, UK. Boeing has also requested that other 787 operators report any similar occurrences worldwide. The DGCA has specifically instructed Air India to verify the stowage integrity of the RAT in all aircraft where the Power Conditioning Module (PCM) was recently replaced. The Ram Air Turbine serves as an emergency backup system, providing hydraulic and electrical power if the aircraft’s main engines fail. It operates by using the aircraft’s forward motion to spin a small turbine that extends from the fuselage underside. This ensures critical flight control systems and avionics remain operational during emergencies, such as dual-engine failures or complete electrical system loss. Notably, the RAT deployed during the June 2025 crash of Air India flight AI171 after a dual-engine flameout at Ahmedabad, resulting in the loss of 260 lives. The recent directive stems from an October 4, 2025, incident involving Air India flight AI117, flying from Amritsar (ATQ) to Birmingham (BHX), where the RAT deployed automatically at around 500 feet during descent. The aircraft landed safely, with all electrical and hydraulic systems reportedly functioning normally. According to the DGCA advisory, “Air India has been advised to review the work package of ‘D’ Check for the actions required in view of the change of PCM module, for any discrepancies.” The regulator noted that the RAT is rarely activated during routine operations and is typically reserved for critical failures, making any uncommanded deployment a serious concern. Air India confirmed that despite the unexpected RAT deployment, all onboard systems were stable throughout the flight. Following the incident, the Federation of Indian Pilots (FIP) requested the DGCA to inspect the electrical systems on all Boeing 787s operating in India. In a letter, FIP President GS Randhawa highlighted that the Aircraft Health Monitoring (AHM) system had detected a fault in the Bus Power Control Unit (BPCU), which could have triggered the RAT deployment. The BPCU is essential for managing electrical power distribution, and any malfunction may activate backup systems. FIP emphasized that this incident points to potential systemic electrical vulnerabilities in the Boeing 787 and called for thorough inspections of all aircraft in the country. Civil Aviation Minister Ram Mohan Naidu stated, “The DGCA is already on the job. Once we have more details, we will coordinate with the relevant stakeholders to ensure preventive measures are implemented and such incidents are avoided in the future.” The DGCA has extended its review beyond Air India, requesting Boeing to provide a detailed report on preventive measures and to share information on similar uncommanded RAT deployments globally in recent years. According to reports, those onboard included Ajay Arora, Deputy Managing Director of Woodpecker Greenagri Nutrients Pvt Ltd; Sumit Sharma of the State Bank of India; Rakesh Tikku, Vice President (Operations); and Manish Kumar Pandey, the company’s Uttar Pradesh project head. Both individuals were uninjured but became stranded in the challenging terrain of the wetlands. They managed to use their cellphones to send their coordinates to emergency responders, enabling rescue teams to locate and extract them safely. Fire rescue units responded to the 12800 block of Southwest 145th Avenue, where the plane had flipped upside down. Both individuals climbed out of the wreckage and stood on the wing while they used their cellphone to send their coordinates to responding officials. They were transported to Miami Executive Airport after being safely removed from the remote area and checked out by paramedics. The student pilot was on a test flight when the aircraft encountered problems that led to the crash. Kentucky, United States: The National Transportation Safety Board (NTSB) has released its final report into the September 27, 2023, crash of a Piper PA-28-161 that killed a flight instructor and student pilot near Whitesville, Kentucky, concluding that the accident was caused by the instructor’s decision to continue flying into an area of severe thunderstorms. Investigators found the airplane suffered an in-flight breakup after encountering extreme turbulence inside convective weather. The Eagle Flight Academy aircraft departed Bowling Green-Woodhurst Airport at 9:55 p.m. for a night cross-country training mission intended to complete the student’s night-flying requirement. The 22-year-old instructor and 18-year-old student were on their first flight together. According to ADS-B data and air traffic control recordings, the aircraft climbed to 4,500 feet and was warned by controllers at 10:44 p.m. of “heavy to extreme precipitation” to its left. Two minutes later, the instructor requested an instrument flight rules clearance but continued circling in the weather and reported the airplane was “getting blown around like crazy” and in “pretty extreme turbulence.” Moments later, communications ceased. Radar data showed a right descending turn before the plane broke apart. The debris field stretched across 25 acres of wooded terrain. Post-accident examination revealed no mechanical failures. Investigators confirmed the aircraft exceeded its structural limits while penetrating severe weather. The NTSB cited the instructor’s decision-making as the defining factor: “The flight instructor’s improper decision to continue flight into a known area of thunderstorms resulted in an in-flight breakup.” The weather briefing obtained before departure included an active convective SIGMET warning of severe storms with tops to 42,000 feet, hail, and wind gusts up to 50 knots. The NTSB also highlighted the potential latency of weather radar images from cockpit and mobile applications, cautioning pilots that such data may be several minutes old and could under-represent the severity or position of fast-moving storms. Autopsies determined both occupants died of multiple blunt-force injuries. Toxicology tests were negative for the instructor. The student tested positive for cetirizine, an over-the-counter antihistamine, but impairment could not be determined.
