Earlier South Korea’s parliament had approved an independent investigation into the December 2024 Jeju Air crash that killed 179 people, responding to delays and criticism of the official probe. Only two cabin crew members survived. Opposition lawmaker Kim Eun-hye released documents from a government-commissioned analysis indicating that the concrete mound should have been designed to be fragile, able to break easily upon impact given its proximity (less than 240 m) to the runway end. Under airport safety operation standards, such structures must be breakable if situated within this safety area to minimize impact forces in runway overruns. The ministry acknowledged that during a 2020 upgrade project, the mound was not modified to meet these criteria. Simulations by the Korean Society of Computational Structural Engineering, commissioned by the Aviation and Railway Accident Investigation Board, found that without the rigid concrete mound the aircraft could have slid to a stop after sliding beyond the runway without sustaining fatal structural damage. A frangible design would similarly have greatly reduced impact severity, possibly allowing the aircraft to breach the perimeter fence and slide into open terrain with no serious injuries expected. The ministry’s admission has reignited criticism from politicians and victim families, with some lawmakers arguing the disaster was largely preventable and that longstanding regulatory oversights were to blame. Opposition figures described the embankment as a “mound of death” and alleged that officials attempted to suppress or delay release of critical safety findings. In parliament, a special committee is examining the crash and broader aviation safety protocols. Rep. Kim Eun-hye has accused the government of negligence dating back decades, asserting that the structure violated both domestic and international safety practices. Families of the victims are demanding greater transparency and accountability. While the simulation report has heightened focus on the airport structure, a final official investigation by the Aviation and Railway Accident Investigation Board is still pending. Prosecutors and police have separately searched offices of Jeju Air and airport operators for documents related to aircraft operations and runway safety features. In the aftermath of the crash, the Transport Ministry and airport authorities have pledged to review and upgrade runway safety areas nationwide, ensuring that navigational aids and nearby structures comply with both domestic regulations and International Civil Aviation Organization (ICAO) standards. Reports indicate plans to remove the concrete embankment at Muan and replace it with breakaway installations at Muan and other regional airports.

JetBlue Flight B6-1112, operated by an Airbus A320, was climbing after departure from Curaçao, a Dutch Caribbean island near the Venezuelan coast, when the flight crew reported a U.S. Air Force air-to-air refueling tanker crossing directly into their flight path at a similar altitude. Air traffic control recordings indicate the commercial jet halted its ascent to maintain separation. In audio released by air traffic control, the JetBlue pilot is heard saying, “We almost had a midair collision up here,” adding that the tanker aircraft had “passed directly in our flight path … they don’t have their transponder turned on.” The absence of a functioning transponder meant the military aircraft was not visible on civilian radar systems, complicating the air traffic controller’s ability to track it. The latest tragedy claimed the lives of 26-year-old Indian trainee pilot Mayank Prajapati and 35-year-old South African pilot Martin Brown, reigniting serious concerns over safety standards, oversight, and accountability in the country’s flight-training industry. Both pilots were enrolled at Aviation Junction Flying School, based in Secunda, Mpumalanga. On 15 October 2025, they were flying separate Sling 2 training aircraft belonging to the flying school, Mayank Prajapati was flying ZU-PBD, while Martin Brown was operating ZU-PBC. He had earned his fixed-wing pilot licence around 18 months earlier through Aviation Junction Flying School. Both aircraft reportedly took off around the same time for routine training operations. Soon after departure, the two aircraft encountered poor weather, including heavy mist, drizzle, and low visibility, resulting in loss of radio and radar contact. Search and rescue teams later discovered the wreckage in separate areas of the KwaZulu-Natal Midlands, Brown’s ZU-PBC was found near Nottingham Road, while Prajapati’s ZU-PBD crashed closer to Pietermaritzburg. Brown’s body was recovered the following day, and Prajapati’s remains were transported to Pietermaritzburg Hospital for post-mortem formalities. The twin crashes have intensified calls for stronger oversight and safety reforms within South Africa’s flight-training industry. A light aircraft en route to Wonderboom Airport lost contact and later was found crashed east of Pretoria (Cullinan). Authorities reported the deaths of the flight instructor and the trainee; initial accounts placed the aircraft on a routine general-aviation flight before the accident. February 2024 – Westonaria, Gauteng Province A Cessna 172K registered ZS-FJU operated by Sun Quest Trading under Part 141 training operations crashed during a training flight. Two occupants were killed, flight instructor and student pilot. While investigations into many of these crashes remain ongoing, aviation-safety experts say the pattern is alarming recurring training flights, small aircraft, and adverse-weather decisions. The South African Civil Aviation Authority (SACAA) has been criticised for reactive oversight and limited enforcement capacity over smaller training schools, especially those operating older piston aircraft. New Delhi, India: The Ministry of Civil Aviation (MoCA) is set to convene a comprehensive review meeting with India’s flying training organisations (FTOs) after the DGCA had announced on July 9 the launch of a bi-annual ranking system for flying training organisations nationwide, effective by 1 October 2025 and then the Directorate General of Civil Aviation (DGCA) released its first-ever ranking report, revealing that none of the country’s training academies met the top performance standards. According to the DGCA’s assessment, covering the period from September 1, 2024, to August 31, 2025, none of the 35 evaluated flying schools achieved an “A+” or “A” grade. The rankings placed 13 FTOs in the ‘B’ category and 22 in ‘C’, indicating widespread gaps in operational efficiency, safety practices, and training quality. The grading framework classified institutions based on their overall performance: A+ (85% and above) A (70–84.99%) B (50–69.99%) C (below 50%) A senior ministry official said the upcoming review will focus on identifying systemic shortcomings and developing corrective measures. “The objective is to improve training quality and ensure that India’s flying schools meet international standards in safety, efficiency, and student outcomes,” the official stated. FTOs placed in the “C” category have been directed to undertake self-assessment exercises and submit improvement plans to the DGCA within a stipulated timeframe. The aviation regulator is expected to monitor compliance closely and provide targeted support to address performance deficiencies. Civil Aviation Minister K. Rammohan Naidu has emphasised that the ranking initiative aims to promote transparency and accountability within the pilot training ecosystem. The DGCA will now publish rankings every six months, with the next report scheduled for April 1, 2026. The government’s move comes at a time when India is witnessing a growing demand for pilots to support the expanding civil aviation sector. Strengthening training infrastructure and ensuring adherence to regulatory standards are seen as key priorities to meet this demand sustainably.
